무증상 갑상선염에 의한 갑상선중독증과 동반된 고칼슘혈증 1예

고칼슘혈증은 흔히 일차성 부갑상선기능항진증이나 악성종양에 의해 발생하는 것으로 알려져 있으며 드물게 갑상선기능항진증에 의해 유발 될 수 있다고 보고되고 있다. 갑상선기능항진증이 고칼슘혈증을 일으킬 수 있는 기전으로는 갑상선 호르몬이 직접적으로 골 흡수를 자극하 거나, 카테콜라민에 대한 베타 아드레날린 수용체 감수성을 증가시켜 골교체율이 증가되는 것 등이 제시되고 있다. 국내에서 갑상선기능항 진증과 동반되어 고칼슘혈증이 발생된 몇 예가 보고된 바가 있으나 갑상선기능항진증이 아닌 일시적 갑상선중독증에 의해 고칼슘혈증이 발생된 예는 문헌고찰상 없다. 이에 저자들은 무증상 갑상선염에 의한 갑상선중독증과 동반된 고칼슘혈증이 갑상선 호르몬 수치의 개선과 함께 호전된 1예를 경험하였기에 보고하는 바이다. 증례: 32세 남자환자가 구토와 설사를 주소로 내원하였다. 신체검사상 전경부에 갑상선종 대나 압통은 관찰되지 않았으며 복부 진찰에서 이상소견은 없었다. 생화학검사에서 AST 80 u/L, ALT 94 U/L, ALP 181 U/L, r-GTP 111U/L, 총 Bilirubin 0.69 mg/dL, 총 Ca 11.3 mg/dL, 이온화 칼슘 6.6 mg/dL, iP 5.5 mg/dL 이었으며 갑상선기능검사는 혈청 T3 2.27 ng/ml, 유리 T4 5.51 ng/dl, TSH <0.01 uIU/ml, TBII 3%, 항티로글로블린 항체 409.9 IU/mL, 항미크로솜항체 <5.0 IU/mL이었다. PTH <1pg/mL, PTH-rp
<1.1pmol/L, 25(OH)VitD 32.7ng/mL였다. 갑상선초음파검사상 특이소견 관찰되지 않았으며 방사선 동위원소 요오드섭취율은 6시간 후 3%, 24시간 후 2%로 감소되어 있어 무증상 갑상선염에 의한 갑상선중독증 및 이와 동반된 고칼슘혈증으로 진단하였다. 고칼슘혈증 치료 및 갑 상선중독증의 증상 개선을 위해 생리식염수 주사와 베타차단제를 투여하였다. 1개월 후 총 칼슘 10.3 mg/dL, iP 4.8 mg/dL, 유리T4 0.69 ng/dL, TSH 0.06 uIU/mL로 호전되는 소견을 보였으며 4개월 후 갑상선기능 및 칼슘은 정상을 보였고 현재 외래추적 관찰 중이다.

A case of pheochromocytoma with nephrotic syndrome as initial manifestation

Pheochromocytoma is catecholamine secreting tumor, which is often diagnosed by its typical symptoms of paroxysmal hypertension accompanied with various signs and symptoms. However, nephrotic syndrome with massive proteinuria is a very rare clinical manifestation of pheochromocytoma. A 59 year-old female patient presented with hypertension and massive proteinuria. On September 2008, she referred to Ajou University Medical Center because of left adrenal gland mass detected incidentally in ultrasonography of abdomen for health examination. She had a mild facial edema, a blood pressure of 165/91 mmHg and a pulse rate of 70 beat/min. Serum chemical values were urea nitrogen 21.2 mg/dl, creatinine 1.1 mg/dl and albumin 2.0g/dl. Urinary protein value was 8.75 g/24hr and creatinine clearance was 52.2 mL/min. Twenty-four hour urine specimen contained 1626 μg of metanephrine (normal : 0-300 μg/day), 9.74 μg of vanillylmandelic acid (VMA) (normal : 0-8 μg/day), 7.0 μg of epinephrine (normal : <20 μg/day) and 36.9 μg of norepinephrine (normal : 15-80 μg/day). Urinary free cortisol, 17-hydroxycorticosteroid and 17-(OHCS) were within the normal range. A contrast computerized tomography (CT) scan of the abdomen revealed 3.5cm sized left adrenal mass. These findings were consistent with the diagnosis of both pheochromocytoma and nephrotic syndrome. For the other tests to exclude the other causes of nephrotic syndrome showed no abnormality, it was likely that pheochromocytoma was the cause of nephrotic syndrome. The patient underwent successful laparoscopic resection of a pheochromocytoma. After the operation, her blood pressure and catecholamine level were normalized, however massive proteinuria was not recovered. The possible mechanism for the nephrotic syndrome might be related to increased clearance of proteins due to high hydrostatic pressure induced by constriction of efferent arterioles or increased permeability of glomerular basement membrane by local hypercoagulative condition. Nephrotic syndrome may be one of the consequences of the exposure to excessive catecholamine of pheochromocytoma, rather than a coincidental event.
