Lactococcus lactis cremoris에 의해 발생한 간농양 및 농흉 1예

배경: Lactococcus lactis cremoris는 조건 무산소성 그람양성알균으로 사람에게 감염을 일으키는 예는 흔하지 않으며 이전에 보고된 인체 감염 예는 흔하지 않다. 또한 Lactococcus lactis cremoris는 가축의 피부 표면에 사는 정상 세균총이기 때문에 가축, 저살균 우유나 유제품에 노출된 경우 감염의 위험요인이 되는 것으로 알려져 있다. 저자들은 이전에 알려진 위험인자를 가지고 있지 않으며 면역력이 정상인 사람에 게서 Lactococcus lactis cremoris 에 의해 발생한 간농양과 농흉 1예를 경험하여 이에 보고하는 바이다. 증례: 과거 특이한 질환이나 가족력 이 없는 42세의 남자 환자가 내원 7일 전부터 시작된 발열 및 우상복부 통증을 주소로 내원하였다. 내원 당시 시행한 이학적 검사상 우하부 폐음이 감소되어 있었고 우상복부에서 압통이 관찰되었다. 내원 당시 시행한 혈액 검사상 백혈구는 17,160/ul로 증가되어 있었으며 AST, ALT, ALP는 정상 범위였으나 total bilirubin 2.2 mg/dL, GGT 152 IU/ml로 증가되어 있었다. 복부 전산화단층촬영에서는 간 우엽(segment 8)에 3.4cm 크기의 간농양이 관찰되었고 흉부 전산화단층촬영에서는 양폐하의 다발성 침윤과 우측 폐의 흉수 및 농흉 의심 소견 관찰되었 다. 흉수 천자상에서는 LDH 635 IU/L, protein 4.9 g/dL, 혈청 protein 7.6 g/dL로 삼출액의 기준에 부합하였다. 내원 당시 시행한 혈액 배양 검사상 Lactococcus lactis cremoris가 동정되었으며 간농양으로부터 배농한 검체에서도 역시 동일한 균이 동정되었다. Lactococcus lactis cremoris가 동정된 후 문진 다시 시행하였으나 이전에 알려진 위험인자는 특별히 관찰되지 않았다. 환자는 내원 당시 경험적 항생제로 ciprofloxacin, metronidazole, cefotaxime을 투여 받았으며 Lactococcus lactis cremoris 동정 후에는 감수성 결과에 따라 cefotaxime을 단독으로 투여하였다. 그러나 이후 환자는 발열 지속되는 등 임상적으로 호전되는 양상 보이지 않았다. 이에 항생제는 levofloxacin으로 변경하였으며 농흉에 대해 VATS decortications 및 배액 시행하였다. 이후 환자는 발열 등 소실되고 임상적으로 호전되는 소견 보였으며 내원 31일째 퇴원 하였다.

Septicemia due to Pasteurella pneumotropica in a patient with cerebral hemorrhage

Pasteurella pneumotropica, a Gram-negative opportunistic pathogen, is usually an animal pathogen, which rarely causes infection in human.
This organism has been isolated from mice, dogs, cats, guinea pigs, and numerous other laboratory animals. Only four cases of Pasteurella species infection were reported before, there has been no report of septicemia due to Pasteurella pneumotropica in Korea. A 54-year-old woman was admitted to emergency room with low blood pressure and low O2 saturation. She had craniectomy because of a history of subarachnoid hemorrhage and aneurysm. After discharge, she was admitted to local clinic. So she was not bited to animals. Blood pressure was 69/42 mmHg, artery blood gas analysis reveals PCO2 54 mmHg, PO2 38 mmHg, O2 saturation 70%. White blood cell count was elevated to 31,300/mm³, C-reactive protein was elevated to 27.8 mg/dL, too. We checked pneumonia in both lungs with abscess formation in left lower lobe on chest x-ray and chest computed tomography. Blood cultures were obtained on admission. The organism was identified as Pasteurella pneumotropica and patient was given intravenous piperacillin/tazobactam. The following up blood culture did not reveal any organism. And then she improved condition and stabilized vital sign.
