「設施隔離」轉「居家隔離」條件放寬新制
□ 設施隔離短期停留外籍人士轉居家隔離條件放寬說明
○ (新制內容) 具備可居家隔離住所、且現居韓國國內的本國籍／在韓長 期居留外籍人士，其三等親*內親屬可轉為居家隔離。
* 包含直系尊親屬(父母), 二等親(兄弟姊妹), 三等親(叔叔、姑姑、舅舅、阿姨、侄兒)

可轉居家 隔離條件
需設施隔離者，若與具備可居家隔離 住所、且現居韓國國內的韓國籍／長 期居留外籍人士為下列任一關係：
① 韓國籍國民的配偶
② 在韓長期居留外籍人士的配偶
③ 韓國籍國民的直系卑親屬
④ 在韓長期居留外籍人士的直系卑 親屬
符合左方①②③④或下方⑤⑥⑦⑧ 任一條件，可轉居家隔離：
⑤ 韓國籍國民的直系尊親屬
⑥ 在韓長期居留外籍人士的直系尊 親屬
⑦ 韓國籍國民的三等親內親屬
⑧ 在韓長期居留外籍人士的三等親 內親屬
○ (確認親屬關係) 僅限持有家族關係證明書等政府開立文件者適用，並由 當事人負舉證責任。
* 如韓國籍配偶已在國外辦理婚姻登記，而未在韓國辦理婚姻登記者，需繳交由外國政府 開立之婚姻／親屬關係證明文件；並檢附經各國經辦機關辦理之海牙認證(Apostille)。
○ (確認符合居家隔離) 非屬配偶或直系尊／卑親屬，而為三等親內親屬 者，需額外檢附提供隔離住所的本國籍／在韓長期居留外籍人士簽署隔 離支援確認書*。
* 提供隔離住所者，因不具備對配偶／直系尊卑親屬以外三等親之扶養責任，故需明確 表示支援其居家隔離之意願與責任歸屬。
○ 設施隔離轉居家隔離者，將同等適用安裝居家隔離app等居家隔離管 理基準。
□ 實施日期 : 自2020年5月21日起實施。

격리대상자는 「감염병의 예방 및 관리에 관한 법률」 제42조, 「검역법」 제
16조에 따라 시설격리 동의서를 작성하고 대한민국에 입국한 시설격리
대상입니다.
본인은 격리대상자에게 상기 격리장소를 제공하고 격리기간 동안 자가
격리 수칙 준수 및 관계기관(보건소·시군구 등)의 모니터링에 적극 협조할
것을 확약합니다.

The quarantined person has entered the Republic of Korea after submitting the Agreement to Facility Quarantine and thus is subject to facility quarantine, in accordance with Article 42 of the Infectious Disease Control and Prevention Act and Article 16 of the Quarantine Act.
I, the guardian, hereby confirm my guardianship for the quarantined person and pledge to provide the place of quarantine to the said person, abide by the guidelines for home quarantine, and fully cooperate with related institutions including local medical centers and city/provincial governments for their monitoring efforts.
