이상의 결과를 바탕으로 다음과 같이 제언한다.

첫째, 본 연구는 중환자실 환자의 경험만을 대상으로 하였으므로 중 환자실 환자 가족 및 질병별 전실 경험 연구를 통해 본 연구에서 확인되 지 않은 다양한 의미를 보충할 수 있는 후속 연구를 제언한다.

둘째, 본 연구 참여자의 전실 경험을 토대로 전실을 경험하는 환자의 정서적 지지를 강화할 수 있는 의사소통 교육이 필요함을 제언한다.

셋째, 중환자실 환자의 전실에 대한 개별적인 신체적, 정서적 경험을 평가하고 중재할 수 있는 전실 전·후 간호 프로그램 개발을 제언한다.

ABSTRACT

-A  Phenomenological Study on the  Experiences of Transfer  from the Intensive Care Unit to the

General Wards.

This study was a phenomenological study conducted for the in-depth understanding and description of experiences of patients transferred from ICU to the general ward by using the phenomenological methodology of Colaizzi (1978). The participants are 15 adult patients between 3 to 5 days after transferred from the medical/surgical ICU where they stayed at least 48 hours at two university hospitals located in Seoul and Chungcheongnamdo from January to December, 2017.

In this study, the participants’ experience of being transferred from the ICU was presented in 8 themes and 4 themes clusters. The 4

theme clusters for patients’ experience of being transferred from ICU were extracted as ‘Hope from despair’, ‘Gratitude for being alive’,

‘Recovery from suffering’, and ‘Pledge to return to living daily life’.

The first theme cluster, ‘Hope from despair’ showed that in the course of planning and preparing for transfer from the ICU, patients consider transfer as a sign of recovering from the disease and hope for life. This was displayed in the themes of ‘message for hope arising from despair’ and ‘a wish to escape from a critical care status’.

The second theme cluster, ‘Gratitude for being alive’ is about the experiences regarding the moment of leaving the ICU after completing intensive care and adapting after being transferred from the ICU, and it shows recovery of daily life and the gratitude for others’ help. This was displayed in the themes of ‘gratitude for the possibility of returning to the life as usual’ and ‘gratitude for the sacrifices of others’.

The third theme cluster, ‘Recovery from suffering” is about the experience of recovery in the course of being transferred from the ICU with spatial change, environmental change, and passage of time and it shows the recovery of physical and mental status. This was displayed in the themes of ’Recovery from the powerless body’ and ‘departure from vulnerability’.

The fourth theme cluster, ‘Pledge to return to living daily life’

means the experiences of reflecting on life in relation to their health condition after being transferred from the ICU and preparing for everyday life that they should return to after being released from

hospital. This was displayed in the themes of ‘reflecting on the times of suffering’ and ‘responsibility of going back to daily life’.

In conclusion, the key phenomenon regarding the patients’

experience of being transferred from the ICU was ‘the dynamic processes of finding hope from pain and despair experienced in the course of intensive care, appreciating survival with the help of many others, and preparing for healthy daily life and recovery of their role beyond treatment-oriented recovery’.

The present study is meaningful in the sense that it could withdraw a comprehensive and practical understanding by analyzing the participants’ experience of being transferred from ICU based on phenomenology. It is hereby hoped that the results of this study will contribute to establish the framework for a desirable and appropriate nursing intervention development to help ICU patients to prepare for transfer from ICU and adapt to life after transfer to a general ward.
