섬유근육통에서 말초 소섬유 신경의 평가

배경과 목적: 섬유근육통은 소섬유신경병증의 특징들과 연관되어 있다. Sudoscan®은 전기화학적인 피부전도도를 이용하여 땀샘 기능장애를 측정하는 비침 습적인 도구이며 말초 소섬유신경기능을 평가하는데 유용하다. 섬유근육통 환자에서 말초소섬유신경의 기능장애에 대해 알려진 것이 적다. 이 논문의 목적은 소신경병증을 가진 섬유근육통 환자와 대조군을 비교하여 유병률과 특징들을 평가하였고 섬유근육통에서 소신경병증과 연관된 인자들을 확인해보고자 한다.
방법: 2010년 ACR 진단기준에 충족하는 66명의 섬유근육통 환자(나이:53±9세)와 66명의 건강한 대조군을 대상으로 하였다. 각각의 섬유근육통환자는 혈액 검사 전 8~10시간 동안 금식하였다. 혈청 공복 혈당, 공복 인슐린, 혈청 25-(OH) D 그리고 75 g 2시간 경구당내성검사를 시행되었다. 키와 몸무게는 신체질량 지수(BMI)로 측정-계산되었다. 현성 당뇨병 환자는 과거력, 투약 병력을 이용하여 제외되었다. 소섬유신경병증 증상 검사 설문조사(SFN-SIQ)는 신경병증 증 상들을 기록하는데 이용되었다. 섬유근육통 영향도 설문조사(FIQ), Beck 우울 검사(BDI), 상태-특성불안 검사(STAI)를 임상적 평가에 포함시켰다. 모든 대 상자들은 Sudoscan®검사를 시행했다. 결과: 대조군과 비교하여 섬유근육통군에서 양쪽 발과 손의 평균 피부전도도 값이 유의하게 낮았다. 평균 손 피부전도 도는 나이, 신체질량지수, 소섬유신경병증 증상 검사 설문조사, 섬유근육통영향도 설문조사, 공복 혈당, 공복 인슐린, 인슐린저항성 지표(HOMA-IR), 비타민 D, 2시간 경구당내성검사와 연관성이 없었다. 하지만 평균 발 피부전도도는 공복 인슐린(r＝ -0.34, p＜0.01), HOMA-IR( r＝ -0.37, p＜0.01)과 의미 있는 상관관계를 보여줬다. 결론: 섬유근육통 환자에서 땀 분비 신경 기능은 대조군과 비교하여 의미 있게 낮았다. 섬유근육통 환자에서 평균 발 피부전도도는 고인 슐혈증과 HOMA-IR과 상관관계 있었으며, 이는 인슐린 저항성 관련 소섬유신경병증이 섬유근육통에 중요한 역할을 할 것임을 암시한다.

A fatal case of adult onset still’s disease complicated by macrophage activation syndrome

Introduction: Adult-onset Still’s disease (ASD) is an inflammatory disorder characterized by quotidian fevers, arthritis, and an evanescent salmon-col- ored rash. Macrophage activation syndrome (MAS) is a fatal reactive condition in which macropahges and lymphocytes are over-activated. Herein, we report a fatal case of ASD complicated by MAS that is refractory to glucocorticoids and immunosuppressive treatments. Case: A 69-year-old woman presented with a 20-day history of intermittent fever, myalgia and general ache. She had been diagnosed as Sheehan's syndrome 40 years ago, and treat- ed with prednisolone (PD) and thyroid hormone. Laboratory tests revealed leukocytosis, hypokalemia of 2.3 mEq/L, low serum renin of 0.2 ng/mL/h, high troponin I of 9.3 ng/mL, high erythrocyte sediment rate of 74 mm/h, high C-reactive protein of 10.698 mg/dL, and high Serum ferritin of 12,885 ng/mL. Rheumatoid factor was 9.8 IU/mL, and antinuclear antibody was negative. Transthoracic echocardiography demonstrated decreased wall mo- tion, pericardial effusion and mild pulmonary hypertension of 39 mmHg. The patient was diagnosed as ASD with myocarditis and hyporeninemic hypoaldosteronism. With PD up to 40 mg a day, fever and general weakness were improved. However, dose dependently, the symptoms were refractory at a lower dose of PD. Despite the use of azathioprine and methotrexate, the patient’s condition was getting worse showing pancytopenia, anisocytosis and poikilocytosis without evidence of any infection. We started intravenous methylprednisolone and cyclosporine under the diagnosis as MAS compli- cating ASD. Nonetheless, the patient died refractorily to the treatments. Conclusions: ASD is usually known as a benign disease of good prognosis, however, recently there are accumulating reports of an association with fatal complications refractory to the conventional treatments. MAS is a life-threatening condition, thus, we suggest that the physicians should be alert to the possibility of MAS complicating ASD because prompt recognition would be critical to control the complication.
