영상학적 검사에서 림프종으로 오인된 회장 말단부의 살모넬라 감염증

서론: 살모넬라 대장염은 Salmonella typhi, Salmonella paratyphi에 의해서 유발되며, 소장의 Payer's patch, 회장 말단부 또는 우측 대장을 잘 침범하고, 직장은 잘 침범하지 않는다. 고열과 전신 쇠약감으로 내원한 환자에서 컴퓨터 단층 촬영(CT) 및 양전자 방출 단층 촬영-컴퓨터 단층 촬영(PET-CT)에서 회장 말단부 및 회-결장 장간막 림프종으로 진단하였으나, 대장 내시경 검사 및 혈액 배양 검사, 위장관액 배양 검사 를 통해 살모넬라 대장염으로 확인된 1예를 보고하고자 한다. 증례: 55세 여자 환자가 고열로 응급실로 전원 되었다. 환자는 내원 일주일 전부터 고열과 전신 쇠약감이 있어 5일간 입원 치료를 하였으나, 호전이 없었다고 하며, 내원 당시 체온은 36℃로 정상이었으나, 백혈구 8650/mm3(림프구 1340/mm3), Hb 10.7 g/dL, 혈소판 141,000/mm3, 알부민 2.7 g/dL, ALT 395 U/dL, AST 1134 U/dL, 총 빌리루빈 0.6 mg/dL, 직접 빌리루빈 0.2 mg/dL, CRP 11.07 mg/dL 이었고, A, B, C, E 형 간염 항원/항체 검사는 모두 음성이었다. 입원 다음 날부터 39℃에 이르 는 고열이 지속되어 복부-골반 CT촬영을 하였다. CT에서는 비장의 종대와 함께 회장 말단부와 회맹판 장관의 균질한 부종, 회-결장 장간막 을 따라 다발성의 임파선 종대가 관찰되었고, 회장 말단부에서 발생한 림프종이라고 진단하였다. 내원 사흘째 날 환자가 선혈변을 호소하여 응급 대장 내시경 검사를 시행하였고, 회장 말단부와 회맹판 및 상행 대장의 근위부에 국한된 다발성 궤양 및 출혈반과 함께 회장 말단부에 서 활동성 출혈이 확인되어, 내시경적 지혈 치료를 시행하였다. 또한 조직 검사와 함께 위장관액과 장관 조직을 채취하여 균배양을 시행하였 다. 림프종에 대한 추가 검사로 PET-CT를 시행하였고, PET-CT 결과 역시 회장 말단부와 회맹판에 강력한 FDG 섭취가 관찰되었고, 주변의 임파선에도 FDG 섭취가 증가하여 림프종을 시사하였다. 그러나 혈액 배양 검사와 위장관액 배양 검사에서 Salmonella paratyphi-A가 검출 되었고, 조직 검사에서는 궤양과 염증 소견 이외에는 특이 소견이 없었다. 종대된 임파선에 대해 초음파 유도하 흡인 조직 검사를 시행하였 는데 림프종을 의심할 만한 조직학적 소견은 관찰되지 않았다. 환자는 격리 조치와 함께 cephalosporin 계열의 항생제로 치료 후, 간기능 검사와 전신 상태 등이 호전되어 퇴원하였다. 결론: 저자들은 영상학적 검사에서 회장 말단부 및 회-결장 장간막 림프종으로 진단하였으나, 대장 내시경 검사 및 혈액 배양 검사, 위장관액 배양 검사를 통해 살모넬라 대장염으로 확인된 1예를 문헌 고찰과 함께 보고하는 바이다.

A case of idiopathic pancreatitis in patient with Crohn’s disease

Pancreatitis has occasionally been associated with Crohn´s disease (CD). A definite etiology can be identified in most patients but a very small proportion remains idiopathic. An example of idiopathic pancreatitis resolved along with the clinical improvement of CD is reported in a 25 years old male patient. His previous history revealed anal fistulectomy two times. He had been diagnosed endoscopically as CD with periumbilical pain and diarrhea 3 years ago. However, further proper treatment was not taken. In our hospital, ileocolonoscopy showed a few, small, longitudinal, active ulcers with normally looking, intervening mucosa at the terminal ileum. The enteroclysis showed ileojejunal multiple nodularities and strictures. Endoscopy of the upper gastrointestinal tract was carried out but no abnormalities were found. The laboratory findings showed elevated serum amylase (951 IU/L) and lipase (326 IU/L) without positive autoantibodies. There was no abnormal finding of pancreas and pancreaticobiliary duct in enhanced pancreatic CT and MRCP. None of the classical etiologies for pancreatitis were found.
With the treatment with antibiotics, 5-aminosalicylic acid, steroid, and azathioprine, as a whole, decreasing pattern and intermittent fine coordinated fluctuation of the levels of amylase and lipase along with the decrease of Crohn’s disease activity index (CDAI) or the CRP level were seen. Then, three months later after the start of treatment, normalization of the pancreatic enzymes was observed without recurrent elevation during the 6 months in maintenance therapy. During the treatment, no adverse effect of medication was observed. This case report supports the concept of an association between idiopathic pancreatitis and CD, a significant and close relation between the level of serum amylase/lipase and CDAI.
