원인을 알 수 없는 위장관내 출혈로 오인된 십이지장 게실 출혈 1례

십이지장 게실출혈은 모든 상부위장관 출혈의 약 0.14%정도를 차지하는 상부위장관 출혈의 드문 원인이다. 십이지장 게실은 대개 상부위장관 내시경 혹은 바 륨검사 도중 우연히 발견되는 경우가 많고, 대부분 무증상이지만, 드물게 게실염, 천공, 폐쇄 혹은 출혈 증의 합병증을 유발하기도 한다. 특히 십이지장 2부의 게실은 특히 해부학적 특성으로 인해 일반 위장관내시경으로 접근이 용이하지 않아, 임상적으로 위장관내 출혈이 의심될 때 출혈 여부를 발견하기 쉽지 않다.
따라서 대량 출혈을 유발할 수 있고, 수술적인 치료를 시도한다 할지라도 수술 술기가 어려워 수술 이후에도 합병증을 동반하는 경우가 많기 때문에, 정확한 내 시경적 진단 및 치료가 필요하다. 저자들은 초기 흑색변 주소로 내원하여 시행한 초기 상부위장관내시경 검사에서 위장관내 출혈의 원인을 발견하지 못하였으 나, 추후 시행한 상부위장관 내시경에서 궤양을 동반한 십이지장 게실 출혈을 진단하고, 성공적으로 지혈 치료한 증례를 경험하였기에 이를 보고한다. 증례 76 세 여자 환자가 흑색변 및 상복통 주소로 응급실로 내원하였다. 환자는 2주 전 흑색변 증상 1회 있어 모병원 진료 후 위장관내 출혈 의심 하에 상부위장관내시 경 및 대장내시경 시행하였으나 출혈 부위 찾지 못하였다. 환자는 천식 외에 특이 약물복용력, 과거력은 없었다. 내원 당시 혈압은 70/50 mmHg, 심박동수 102 회/min,호흡수 20회/min,액와 체온 36도, 산소포화도 95% 측정되었으며, 18Fr 경관 세척 상 짙은 갈색의 배액 양상 보였다. 신체 진찰에서 결막은 창백하였 고, 경미한 상복부 압통 외에는 특이 소견 없었다. 검사실 소견에서 백혈구 7600/mm3. 혈색소 7.2 g/dL, 혈소판 184000/mm3, 아밀라아제 339 U/L 리파아제 1178 U/L C 반응 단백 0.70 mg/dl 총단백 6.2 g/dl 알부민 2.5 g/dl AST 17 U/L ALT 6 U/L 총빌리루빈 0.24 mg/dl 크레아티닌 0.57 mg/dl Na 139 mEq/L, K 3.8 mEq/L, Cl 110 mEq/L 측정되었다. 내원 당시 복부 CT 시행하였고, 십이지장 2부와 3부에 각각 6.5 cm, 4.5 cm 크기의 혈종을 동반한 게실이 관찰되었 다. 급성 췌장염 소견은 관찰되지 않았다. 환자는 응급으로 적혈구 수혈 시행하였고, 위장관내 출혈 감별 위한 상부위장관 내시경을 시행하였다. 십이지장 2부 에서 게실이 관찰되었고, 혈종 제거 후 게실내에 6 mm 크기의 혈관이 노출된 타원형 궤양(A1)이 관찰되었으며, 고장성 식염수 주입 및 혈액응고치료를 시행 하였다. 다음날 추적 내시경을 시행하였고 출혈 소견은 관찰되지 않았다. 환자는 이후 금식 유지하며 복통 및 혈색소 수치 호전 되었고 식이 유지후에도 증상 악화 보이지 않아 입원 8일째 퇴원하였다.

Capsule Endoscopy with Retention of the Capsule in a Duodenal Diverticulum: A Case Report

Capsule endoscopy (CE) is a noninvasive, reliable diagnostic tool for examination of the small intestine. But it has many disadvantages including the risk of capsule retention (2.6%). We report a case of capsule retention by small bowel diverticulum which removed endoscopically. A 69-year-old male was admitted to our hospital complaining of a five-day history of hematochezia about 30 mL per day without abdominal pain. We planned EGD, colo- noscopy and CT angiography revealed the second degree internal hemorrhoids and diverticulum in the proximal third duodenal portion of size 4.5 cm.
We performed CE to diagnose the obscure small intestinal bleeding. At three hours after ingestion, the capsule stopped in the small intestine without significant active bleeding lesions. In CT images showed the capsule located in a duodenal diverticulum (Fig. 1). We planned EGD and found the cap- sule in diverticulum orifice and removed easily using the net (Fig. 2). A small bowel series was performed immediately. The transit time was about six hours, and we founded malrotation of the small bowel without bowel obstruction (Fig. 3). The patient was diagnosed as having internal hemorrhoid bleeding and fully recovered. In conclusion, CT is the best tool for locating retention sites before CE. A small bowel series is also useful to check gas- tric and small bowel transit time. When transit times are prolonged, CE may be contraindicated and be replaced with wired enteroscopy. As in this case, EGD may be helpful when the retention site is near the proximal duodenum.
