Diverticulum (게실): 근육막을 통해 장점막층이 탈장되거나 바깥으로 돌출되어 나온 것
Diverticulitis (게실염):
1) Inflammation caused by food or feces trapped in a
diverticulum; may lead to bleeding, perforation, peritonitis, and bowel obstruction
2) Most commonly occur in the sigmoid colon
3) Risk factors: low-fiber diet, constipation, obesity ( 장관내 압 력의 증가) / age, 장근육 허약
Sign & Symptoms:
: pain, nausea, altered bowel habit (diarrhea and constipation), abdominal distension, mild fever

게실질환: Diverticulitis (게실염)
진단:
1) 직장 지두검사 시 덩어리 촉진, 대변검사 시 출혈 여부, CT로 게실 과 농양확인, 바륨관장은 염증이 있는 동안 금기
2) High-fiber diet
3) NPO during acute diverticulitis
4) Medication: analgesics (morphine sulfate is avoided because it
can increase intracolonic pressure), antibiotics, anticholinergic5) Fluid and electrolyte replacement
6) Surgery: hemicolectomy, temporary loop colostomy and removal of involve bowel

진단:
:직장 지두검사 시 덩어리 촉진, 대변검사 시 출혈 여부, CT로 게실과 농양 확인, 바륨관장은 염증이 있는 동안 금기
Management:
1) High-fiber diet
2) 변비 예방 (수분섭취), 비만일 경우 체중감소 3) NPO during acute diverticulitis
4) Medication: analgesics (morphine sulfate is avoided because it can increase intracolonic pressure), antibiotics, anticholinergic, laxative and stool softeners
5) Fluid and electrolyte replacement
6) Surgery: 낭을 결찰하고 제거, 농양이 생겼다면 침범된 장을 절제
(hemicolectomy, temporary loop colostomy and removal of involved bowel)

Etiology
1) constipation 2) Pregnancy

4) Tumor in abdominal cavity
5) Standing or sitting for prolonged periods

Sign & symptom
: 배변시 출혈, 통증, mass (외치핵: 눈으로 보임, 내치핵: 수지 촉진이나 직장경)
Management
1) 변비예방: high-fiber diet, exercise, laxative, 수분섭취 2) 통증완화: 좌욕, 진통제
3) Surgery: 치핵절제술, 결찰…
4) 수술 후 간호
• 수술 직후 prone position 또는 측위
• 치유증진-항문을 깨끗이 하고 좌욕 (정규적, 배변 후)
• 수술 직후를 제외하고 혈액순환을 돕기 위해 국소 습열 제공
• 통증완화, 변완화제 투여, 교육 (장 규칙성, 수분과 고섬유질 섭취 등)
