결론

뇌졸중은 발병 이후 높은 장애 발생률과 합병증으로 의료비 부담이 높은 질환으로 이에 대한 추후관리가 필요한 질환이다. 건강보험심사평가원에서는 뇌졸중 진료의 질적 수준을 지속적으로 평가하여 요양기관들의 질적 차이를 줄이고, 균형적이고 지속적인 질 향상을 도모하고자 급성기 뇌졸중 평가를 실시하고 있다. 현재 급성기 뇌졸중 평가에서는 구조와 과정지표만을 대상으 로 평가하고 있으며, 2010년 평가결과, 대부분 의료기관의 과정지표가 90%

이상으로 향상되었다. 그러나 궁극적으로 의료서비스의 질은 사망과 합병증 발생 등 건강결과의 감소로 평가하게 된다는 점, 의료기관 간에 입원일수와 진료비의 변이가 커지고 있어 이에 대한 관리가 요구된다는 점에서 결과지 표 평가가 필요하다.

건강보험심사평가원에서는 뇌졸중 평가결과를 토대로 가감지급사업을 계 획하는 등 의료서비스의 질 향상을 위한 사업을 수행하고 있다. 이 연구에서

는 뇌졸중 의료서비스의 효율적인 질 관리와 가감지급사업 기반을 마련하기 위하여 적용 가능한 결과지표를 제시하고, 평가기간, 자료수집, 중중도 보정 등 결과지표 평가 방안을 제시하였다.

결과지표 평가는 의료의 질 향상을 목적으로 하며, 최근 뇌졸중 평가에서 질 향상의 의미는 응급으로 입원한 후 생존여부에만 초점을 두는 결과보다 퇴원 이후의 기능 및 활동능력 등에 중점을 두고 있다. 따라서 급성기 뇌졸 중 평가에서는 단기적으로는 사망, 입원일수 등과 같은 결과지표에서 평가하 고, 퇴원 후 기능상태, 장애율과 같은 결과지표로 점차 확대될 필요가 있다.
