Fibromyalgia syndrome in patients with cirrhosis

2Background: Chronic pain is common in patients with liver disease. This study aimed to investigate the prevalence and risk factors of the fi- bromyalgia syndrome in patients with liver cirrhosis. Methods: Patients with liver cirrhosis were prospectively enrolled from the hepatology clinic at Uijeongbu St. Mary’s Hospital. We measured Modified 2010 American College of Rheumatology Diagnostic Criteria for fibromyalgia and Fibromyalgia Impact Questionnaire (FIQ). Logistic regression analysis was used to evaluate factors associated with fibromyalgia syndrome. Results: A total of 112 patients were recruited (mean age 54.6 years, 30.9% women, mean Child-Pugh score 6) of whom 9 (8.0%) met criteria for fibromyalgia. In univariate analysis, age, sex, alcohol drinking, smoking, Child-Pugh score, and etiology of liver disease were not significantly associated with fibromyalgia. Conclusions: This study demonstrates a significant proportion of fibromyalgia syndrome among patients with cirrhosis. The association between fibromyalgia and liver cirrhosis is warranted. A case of ascending colon variceal bleeding with emergent endoscopic variceal obturation

Ectopic varices are extremely rare among the patients with portal hypertension, especially in the ascending colon. It is difficult to evaluate the massive lower gastrointestinal bleeding in the patients with liver cirrhosis by colonoscopy because of hemodynamic instability and poor bowel preparation. In Korea, only one case about ascending colon variceal bleeding was reported, in which hemostasis was performed by venous coil embolization. We report another rare case of ascending colon variceal bleeding in the patient with cirrhosis related to chronic viral hepatitis B, and successfully treated by two sessions of N-butyl-2-cyanoacrylate injection through colonoscopy. This case suggests that the careful endoscopic approach and hemostasis with glue might be an option for the massive bleeding from lower gastrointestinal varix.
