Filling out e-Form (Visa) for Visa Applications
Starting from 1 June 2022, e-Forms (Visa) are required to be filled out on the Korea Visa Portal (www.visa.go.kr) for all visa applications and the previously used ‘Visa Application Form’ will not be accepted anymore. Please complete the e-Form (Visa) online in advance as no entry would be allowed to our office for applications without the completed e-Form.
※ Please refer to the attached manual (File 1) for how to fill out the e-Form (Visa) online.
□ How to fill up the e-Form for Visa Applicants (Non-Visa Issuance Confirmation Number holders)
STEP 1
ㅇ Complete the e-Form (Visa) and submit it to the Korea Visa Portal (www.visa.go.kr).

ㅇ Please fill out the e-Form either in ENGLISH or KOREAN.
ㅇ You are required to fill out ALL fields on the e-Form.
ㅇ For the nationality column, HKSAR passport holders should choose ‘CHINA P. R. (HONG KONG)’ and HKDI (Hong Kong Document of Identity) holders should choose ‘HONG KONG D.I..’
ㅇ For the national identity no. column, please put down Hong Kong or Macau ID number.
ㅇ Please DO NOT fill out the ‘Details of Visa Issuance Confirmation’ part.
ㅇ Please put down full detailed address in the ‘street’ column.
ㅇ For married applicants, please put down your spouse’s information for the marital status details part.
ㅇ After submission, DO NOT CLOSE THE WINDOW / TAB.
If you do, you may not be able to print the e-Form.
ㅇ Once you submit the e-Form and close the window, you cannot edit or rewrite the form. You have to fill out a new e-Form once again for any changes.
STEP 2
ㅇ After completing your e-Form, print it out and sign on it (parents may sign on behalf of underaged children) before submitting it to the Visa Section along with other required documents and fees. (*BY RESERVATION*)

ㅇ Your application number shown on the e-Form should begin with
‘HKXXONXXXXXX’ and it will look like this:
ㅇ An incomplete/incorrect form may cause rejection or delay of your visa application.
ㅇ The photo uploaded on Korea Visa Portal must be the same as the one submitted to the Visa Section.
If you use different photos, your application may be rejected or delayed.
ㅇ After visiting our office for visa application submission, the e-Form submitted cannot be cancelled.

□ How to re-print the e-Form for Visa Applicants
STEP 1
ㅇ Search and check the status of the submitted e-Form by inputting relevant information as shown below.
- http://www.visa.go.kr > English > Check Application Status > Check Application Status & Print
STEP 2
ㅇ To re-print the submitted e-Form, click ‘Next‘ to go to step 2 and click
‘Visa Application‘.
You will then get a .pdf file of your completed e-Form.

電子申請表(簽證)填寫指引
由2022年6月1日起，所有簽證申請人申請簽證前必需填妥於韓國簽證網 (www.visa.go.kr) 上的 ‘e-Form (Visa)’ 電子申請表(簽證)，本領事館將 不再受理舊有的紙本申請表格‘Visa Application Form’。請事先在網上填 妥電子申請表(簽證)，否則將不能進入本領事館。

步驟一
ㅇ 填妥‘e-Form (Visa)’電子申請表(簽證)並提交至‘Korea Visa Portal’
韓國簽證網(www.visa.go.kr).

ㅇ 請以英文或韓文填寫電子申請表。
ㅇ 申請人必需完整填妥整份電子申請表。
ㅇ 在‘Nationality’國籍一欄,
香港特區護照持有人請選擇‘CHINA P. R. (HONG KONG)’;
簽證身份書 (HKDI) 持有人請選擇 ‘HONG KONG D.I.’
ㅇ 在‘National identity no.’國家身份證號碼一欄，請填寫香港或澳門身 份證號碼。
ㅇ 請不要填寫‘Details of Visa Issuance Confirmation’簽證發給認證 書部分，只需將以下四欄漏空。
ㅇ 請在‘Street’街道一欄填寫完整詳細地址。
ㅇ 已婚人士請在‘Marital Status Details’婚姻狀況部分填寫配偶的資料。
ㅇ 按下‘Submission’提交申請表後, 切勿關閉該網頁視窗 若關閉了該網頁視窗，申請人可能無法再列印該電子申請表。
ㅇ 一旦提交電子申請表並關閉網頁視窗，將不能再更改或重新填寫申請表。
若要作出任何更改，申請人需重新填寫新的電子申請表。
步驟二
ㅇ 填妥電子申請表後，請列印該申請表，並在上方簽名(父母可為未成年子女 簽署)，然後連同其他所需文件、近照(證件照)及申請費用前往領事館辦理簽 證申請手續。(*預約到訪*)

ㅇ 電子申請表號碼將以‘HKXXONXXXXXX’開頭(如下圖所示)。
ㅇ 若未填妥或提交了不完整的電子申請表，該簽證申請可能會被拒絕。
ㅇ 上載至‘Korea Visa Portal’韓國簽證網的照片必需與提交至領事館的照 片一致。
若提交了不同的照片，該簽證申請可能會被拒絕。
ㅇ 前往領事館辨理簽證申請手續後，將無法取消已提交的電子申請表。

□ 如何重印簽證電子申請表
步驟一
ㅇ 在‘Korea Visa Portal’韓國簽證網按以下步驟輸入相關個人資料搜尋已 提交的電子申請表。

步驟二
ㅇ 按‘Next’(下一步) 到‘Step 2’(第二步)，
然後按下‘Visa Application’(簽證申請書)。
你已提交的電子申請表將以.pdf檔案形式自動下載。
