대망에 발생한 IgG4 관련 염증성 가성종양 1예

염증성 가성종양은 만성 염증 세포의 침윤 위에 섬유아세포나 근섬유아세포의 증식을 특징으로 하는 병변이다. 근래에는 염증성 가성종양 중 IgG4 양성 형질세포가 풍부한 염증성 가성종양은 전신성 IgG4 연관 경화성 질환과 관련된 질환으로 보며 섬유아세포나 근섬유아세포의 증식을 특징으로 하는 염증성 가성종양과 구별하자는 보고가 있다. 대망에만 국한된 염증성 가성종양의 보고는 아주 드물며 특히 대망에 발생한 IgG4 관련 염증성 가성종양의 보고는 국내에 없기에 보고하는 바이다. 44세 남자가 3일간의 좌측 복통을 주소로 1차병원 내원하였고 시행한 복부초음파상에 좌측 복부에 덩어리가 있어 본원으로 의뢰되었다. 환자는 복부 전산화단층촬영과 전신 양전자방출단층촬영을 시행하 였고 좌측 대망에 국한된 5×6 cm의 악성종양이 의심되어 수술적 치료를 시행하기로 하였다. 술전 시행한 검사에서 암배아항원(CEA)이 9.8로 상승해 있는 것 이외에 혈액검사,방사선검사, 위,대장 내시경 검사에서 특이 소견 없었다. 복강경하에 소장 장간막에 유착되어 있는 덩어리를 박리하여 제거하였으며 임파선 제거는 시행하지 않았다. 조직학적 검사 결과 종양은 섬유화의 바탕 위에 방추형 세포(spindle cell)의 증식이 관찰되며 고배율(×400)에서 50개 이상의 IgG4 양성 형질세포가 관찰되었다. 조직에서 IgG4와 IgG의 비는 증가되어 있으며(30%), 폐쇄성 정 맥염의 동반이 관찰되어 IgG4 관련 염증성 가성종양으로 진단되었다. 환자는 수술을 통한 조직적 진단 후 혈청에서 항핵항체와 IgG4검사를 시행하였고 결과는 음성, 정상이었다. 환자는 합병증 없이 퇴원 후 현재 외래에서 5개월째 경과 관찰 중이다.

충수돌기에 발생한 암육종 (Sarcomatoid carcinoma) 1예

A sarcomatoid carcinoma or carcinosarcoma is very rare biphasic tumor characterized by a combination of malignant epithelial and mesenchymal cells. Our case was a-51-year-old female, with palpable mass on rt. lower abdominal lesion. At first, we checked her abdominal CT &diagnosed mucocele /c adenocarcinoma on rt. colon. Then, laparoscopic hemicolectomy &lymph node dissection was performed. But histollogically, the tumor was Sarcomatoid carcinoma, arising in mucinous cystadenocarcinoma in the appendix. The mass size was 9.5cm length approximately
&infiltrated the appendix. Immunohistochemically, the tumor cell showed a positive immunoreactions for vimentin &pancytokeratin. It means the tumor has two potential cells; epithelial and mesenchymal cell elements. Scarcomatoid carcinoma of colon is malignant tumor with poor prognosis. The patient was treated with doxorubicin &cisplatin therapy, but we lost the patient. A sarcomatoid carcinoma of colon is extremely rare tumor. We found one case &present it with review of the literatures.
