요붕증을 동반한 분만 직후 발생한 쉬한증후군 1예

쉬한 증후군은 출산 후 심한 출혈과 저혈압으로 인해 뇌하수체 기능저하증을 일으키는 질환이다. 발생기전은 아직 명확하지 않으나 뇌하수 체에 혈류를 공급하는 동맥 혈관의 경련과 저혈압에 의해 임신 기간 중 커진 뇌하수체의 동맥 혈류가 저하되기 때문으로 생각되고 있다.
하지만 뇌하수체 후엽은 문맥계가 아닌 하하수체동맥으로부터 혈액을 공급 받기 때문에 일반적으로 거의 손상을 받지 않으나 드물게는 후엽 도 심하게 파괴되어 요붕증을 유발하는 경우도 있다. 일반적으로 쉬한증후군이 서서히 진행된 경우는 잘 알려져 있으나, 급성으로 진행한 경우는 흔하지 않으며, 특히 요붕증을 동반한 경우는 매우 드문 것으로 알려져 있다. 저자 등은 분만 후 출혈성 쇽에 의해 급성으로 발생한 쉬한증후군 환자에서 요붕증이 동반된 1예를 문헌고찰과 함께 보고하는 바이다. 34세 여환은 임신 40 주에 질신 분만 중 태아의 상태 악화 로 응급 제왕절개술 시도 후 자궁수축부전증으로 대량출혈이 있어, 재수술 중 혈압이 촉지되지 않아 본원 응급실로 전원되었다. 농축적혈구 35 단위, 수액보충, 승압제 투여 후 전자궁 적출술을 시행하였다. 수술 후 급성 신부전, 폐부종, 범발성 혈액응고 장애로 중환자실로 이송되 었고, 인공호흡기를 2 주간 치료하였으나 의식 저하 및 근육쇠약감으로 자발호흡이 어려웠다. 뇌하수체 기능저하증이 의심되어 호르몬 검사 를 시행하였고, 하이드로 코르티손 투여 후 환자 의식 및 근육 쇠약감이 호전되어 인공호흡기 중단 후 자발 호흡이 가능해졌다. 뇌하수체 자기 공명 검사에서 뇌하수체 전엽에 출혈이 관찰되었다. 갑상선호르몬과 프레드니솔론 경구 호르몬제로 변경 후 환자는 퇴원하였다. 퇴원 후 2개월에 다뇨증을 주소로 내원하여, 수분제한 검사를 시행하였다.수분제한 3시간후 측정한 요삼투압은 98mOsm/kg이었고 pitressin 5단위 피하주사후 요삼투압이 272mOsm/kg로 요비중이 증가하여 중추성 요붕증으로 진단하였다. 환자는 프레드니솔론, 갑상선호르몬과 함께 바소 프레신 비강 투여로 증세가 호전되어, 현재 외래에서 경과 관찰 중이다.

Reversible pituitary hyperplasia due to profound hypothyroidism related to Hashimoto's thyroiditis

Objective: Pituitary hyperplasia associated with untreated primary hypothyroidism in adolescent may cause unnecessary surgical treatment Case report: We describe an unusual association of pituitary hyperplasia with hypothyroidism. Evidence synthesis: A 15-year-old boy presented with headache and short stature. Brain MRI revealed a pituitary mass. Neurological examination showed intact reflexes and no visual field defects.
He was pre-pubertal. He was in less than the 3rd percentile for height. Results: of serial endocrinologic parameters showed that severe primary autoimmune hypothyroidism and growth hormone deficiency were present. His TSH level was higher than 50m IU/mL and fT4 was extremely low, at 0.05 ng/dL. Random GH was less than 0.01 and IGF-1 level was 96.67 ng/mL (normal >202 and <967). Goiter was noted and his anti-peroxidase antibody level was elevated (>3000U/mL, normal<60). The patient began oral levothyroxine (100 ug a day) and growth hormone replacement. The patient became clinically and biochemically euthyroid and underwent repeated MRI after 3 months of treatment and showed a dramatic reduction in the size of the pituitary tumor. His height initially increased to that of the 10th percentile, and continued to increase along the 50th percentile. Conclusion: This case illustrates that the diagnosis of pituitary hyperplasia secondary to hypothyroidism relies on a thorough history and endocrinologic evaluation. Examination of TFT in patients with pituitary masses may prevent unnecessary operations.
