뇌하수체기능 저하증과 중추성 요붕증을 동반한 재발성 미만성 대세포 B 림프종 1예

중추신경계 림프종은 드문 질환이며 90%가 미만성 대세포 B 림프종으로 알려져 있다. 미만성 대세포 B 림프종의 경우 처음 진단 당시 중추 신경계를 침범하는 빈도는 1%이나 오히려 완전관해 이후 중추신경계 재발의 빈도가 더 높다. 중추신경계 림프종 대부분은 뇌실질에 발생하 며 시상하부-뇌하수체 축을 침범하여 뇌하수체 전엽 기능저하 및 요붕증을 일으킨 예는 매우 드물다. 저자들은 미만성 대세포 B 림프종 4기 진단 후 항암요법(R-CHOP 요법)을 시행받고 완전관해 판정 후 시상 및 시상하부의 재발성 림프종에 의해 뇌하수체 전엽 기능저하 및 중추 성 요붕증이 발생한 1예를 보고하는 바이다. 57세 남자는 뼈, 간, 양측 신장에 미만성 대세포 B 림프종 4기를 진단받고 항암요법(R-CHOP 요법)을 6회 시행 받은 후 완전관해 판정을 받고 추적관찰 중 7개월 뒤 전신쇠약, 보행이상, 의식저하 주소로 응급실 통해 입원하였다. 생화 학검사 결과 고삼투압성 고나트륨 혈증 보였고, 면역혈청검사 결과 스테로이드 호르몬, 갑상선 호르몬, 성 호르몬, 성장 호르몬, 항이뇨 호르 몬 모두 감소하였으며 프로락틴만 약간 상승하였다. 뇌자기공명영상에서 시상 및 시상 하부의 종양 소견 보였고 양전자방출단층촬영 (PET-CT)에서 시상 및 시상 하부 외에 림프절들과 간, 양쪽 신장의 FDG 섭취 증가는 보이지 않아, 중추신경계 단독 재발로 판단되었다. 뇌척 수액 검사 및 조직검사, 고용량 항암요법을 계획하였으나 환자의 의식 및 전신상태를 고려, 시행하기 어렵다고 판단되어 방사선요법 시행하 며 스테로이드 호르몬, 갑상선 호르몬, 항이뇨 호르몬을 보충하였다. 증상 호전되었고 양전자방출단층촬영 추적관찰 상 뇌 병변 호전된 상태 로 추후 전신 항암요법 계획 중이다.

Impact of hyperglycemia on survival and infection in patients with metastatic colorectal cancer

Background: Convincing evidence has shown that individuals with diabetes mellitus are at higher risk of colorectal cancer (CRC). Moreover, diabetic patients with nonmetastatic CRC were found to have poorer overall survival, compared with that of nondiabetic patients. However, the effect of hyperglycemia on survival after metastatic CRC has never been assessed. Therefore, we assessed the impact of hyperglycemia on surviv- al and infectious complications in patients with metastatic CRC receiving a palliative chemotherapy. Patients and Methods: We reviewed the records of 160 patients with newly diagnosed metastatic CRC, treated with palliative chemotherapy from March 2005 to December 2011 in Chungbuk National University Hospital. The mean glucose was calculated for each patient using all available glucose results. Results: The me- dian age was 61 years (range, 16-84), and 37 patients (23.1%) reported diabetes mellitus before metastatic CRC diagnosis. The mean (± SD) glucose was 132.0 ± 40.1 mg/dL and the mean number of glucose examination was 20.5 ± 13.5. Ninety-seven patients (60.6%) had normal range of the mean glucose level (≤ 125 mg/dL), and 63 patients (39.4%) had high mean glucose level (≥ 126 mg/dL). The median overall survival was 21.4 months (95% CI, 16.4-26.4) among the patients with normal mean glucose level and 18.6 months (95% CI, 14.5-22.8) among patients with high mean glucose level, but were observed without statistical significance (p=0.132). The following incidence of infection-related adverse events was more frequent in patients with high mean glucose level than those its normal mean glucose level: any grade (in 17.5 % vs. 39.7%, p=0.002) and grade 3 or 4 (in 15.5% vs. 28.6%, p=0.037). Conclusion: In patients with newly diagnosed metastatic CRC, hyperglycemia was not associated with shorter survival, but hyperglycemia was associated with the incidence of infection and serious infection-related adverse events in patients with metastatic CRC receiving palliative chemotherapy.
