The Influence of Diabetes Mellitus on Diastolic Function in Patients with Non-Ischemic Left Ventricular Systolic Dysfunction

Background: Diabetes mellitus (DM) is known to be a strong risk factor of cardiovascular disease. However, there were limited data about the impact of DM on diastolic function especially in patients with LV systolic dysfunction. The aim of this study is to evaluate the influence of DM on diastolic dysfunction in patients (pts) with non ischemic left ventricular systolic dysfunction. Method: A total of 182 consecutive pts who had non ischemic left systolic dysfunction were enrolled from May 2004 to June 2010. The pts with myocardial infaction, moderate to severe valvular dysfunction, or chronic renal insufficiency were excluded. The patients were divided into two groups according to existence of DM (DM group: n=76, non DM group: :n=106). Baseline clinical and echocardiographic characteristics were compared between two groups.

Results: Baseline clinical characteristics showed that the incidence of hypertension (65.8% vs. 34.9%, p<0.001) and hyperlipidemia (28.9% vs.

12.3%, p=0.007) were higher in DM group compared with non DM group. Baseline echocardiographic characteristics revealed that there was no significant difference in left ventricular ejection fraction (LVEF) and left ventricular mass index. Doppler echocardiographic data showed that E/E’ (20.31±10.02 vs. 16.58±8.23, p=0.022) and E’ (0.037±0.013 vs. 0.046±0.019, p=0.002) were higher in DM group compared with non DM group. Conclusion: In this study, DM is associated with hypertension and hyperlipidemia and diastolic dysfunction including E/E’

and E’ in pts with non ischemic left systolic dysfunction. As the result, we assumed that the DM pts who had non ischemic left systolic dysfunction may have higher major cardiac adverse events and the intensive treatment may be needed.

횡문근융해증(Rhabdomyolysis)에 동반된 급성심근경색(AMI)

서론: 횡문근융해증은 여러 원인으로 골격근 조직이 파괴되어 세포내 독성대사물질이 순환기내로 분비됨으로써 발생하는 여러 증상들의 임 상증후군을 말한다. 진단은 병력 및 근육통, 암갈색 소변 등 임상소견시 의심하고, 급성심근경색이나 뇌혈관질환 없이 혈중 CK가 1,000IU/L 이상 증가 또는 혈중 AST, LDH의 상승 및 혈액과 뇨의 근색소를 증명함으로써 확진할 수 있다. 횡문근융해증의 대표적 합병증인 급성신부 전 외 전해질장애, 구획증후군, 드물게 심정지 및 부정맥이 동반될 수 있다고 하나 급성심근경색이 동반된 경우는 드물다. 본원에서 최근 횡문근융해증 치료 중 갑작스런 급성심근경색이 동반된 1예를 경험하였기에 보고하는 바이다. 증례: 71세 남자 20년 전부터 당뇨, 고혈압으 로 약물 복용 중인 자로, 집에 쓰러져 있는 것 발견되어 응급실 내원하였다. 내원시 의식상태 명료, 활력 징후는 안정적이었고 전신무력감, 암갈색의 진한 소변 보이나 흉통은 없었다. 뇌 자기공명영상 및 심전도의 이상 소견은 없었다. 혈청 생화학검사에서 AST 269U/L, LDH 1676U/L, 혈중 myoglobin 9531 ng/mL, Cr 2 mg/dL, CK 12260 IU/L, CK-MB 310 U/L, troponin-T 양성, Troponin-I 1.62 ng/mL 소견 보였다.

횡문근융해증에 동반된 급성신부전 진단 하에 중환자실 입원하여 수액치료 및 활력징후 모니터링 하였다. 입원치료 3일경 AST 116 U/L, LDH 1597 U/L, 혈중 myoglobin 352 ng/mL, Cr 1 mg/dL, CK 2108 IU/L, CK-MB 310 U/L로 호전 보이나, 갑작스런 호흡곤란 및 흉통 발생 및 심전도 모니터링상 lead II에 ST상승 보였다. 흉부 X-ray상 폐부종, 심전도상 V1-4 ST상승 보여 급성심근경색으로 응급 관상동맥 조영술 시행하였고, 좌전하행지 중간부에서 혈전을 포함한 완전 폐색 보여 관상동맥 중재술 시행하였다. 시술 후 환자 증상 호전되어 퇴원 후 현재 외래 추적 관찰 중이다. 결론: 본원에서 횡문근융해증 및 급성 신부전으로 입원 치료 중 발생한 급성 심근경색에 대해 관상동맥 조영술을 통한 중재적 시술로 치료한 1예를 경험하였기에 보고하는 바이다.
