Physicochemical property and skin damage of physical mixture of valsartan and polysorbate 80

ABSTRACT −To investigate the interaction and skin damage of valsartan with polysorbate 80, the physical mixture of val- sartan and polysorbate 80 was prepared and then its adhesion, dispersibility, DSC and skin damage in nude rats were inves- tigated. The physical mixture of valsartan and polysorbate 80 appeared as an aggregated form and could hardly be dispersed in water. The DSC curve showed that physical mixture disappeared the intrinsic peaks of valsartan and polysorbate 80 at about 115oC and 170oC, respectively. It appeared a new relatively broad endothermic peak at about 150oC, suggesting that valsartan was chemically interacted with polysorbate 80. Furthermore, it induced the severe skin irritation and damage in nude mice. Thus, polysorbate 80 must not be used in the preparation of valsartan-loaded pharmaceutical dosage forms.

발사탄 [Valsartan, (S)-N-valeryl-N-{[2¢-(1H-tertrazol-5- yl)biphenyl-4-yl-metyl}-valine] (Figure 1a)은 Angiotensin II
Receptor의 포화적이고 가역적 억제를 통하여 혈관 수축을
억제하여 혈압을 조절하는 약물로서 다른 고혈압보다 부작 용이현저하게적어노인성고혈압에많이사용하고있다.1,2)
또한, 이약물은약 80%가대사되지않은상태로뇨를통해
배설되며, 구조적으로 비슷한 로사르탄 (losartan)보다 Angiotensin I 억제효과가 적은반면 이뇨제및 기타고혈
압에 사용되는 약물들과 상호작용이적어서 최근 병용투여
로낮은용량에효율적인치료제로주목받고있다.1~5) 제품
으로는노바티스에서 DiovanR (valsartan 80 mg 함유)이라는
상품명으로심부전및심혈관질환치료에도광범위하게효 과가 있다고 알려져 있으며 ARB (Angiotensin Receptor Blockers)계열약물중가장많이선택되는약물이다.5,6) 또
한, 이 약물은 물에서의 용해도(3 ug/ml) 및 생체이용률
(AUC 23%)이낮아 용해도 및 생체이용률을증가시키고자
하는연구가 많이시도되고있다.7~9)
Polysorbate류 (Polyoxyethylene sorbitan fatty acid esters)
(Figure 1b)는계면활성제분류에속하며비이온성계면활성

안널리사용되어왔다. 이첨가제는인체에대한안정성또
한 확보가되어 있어난용성 약물의 용해도를높이기 위해 일차적으로고려되는계면활성제이며 polysorbate 80 (폴리소
르베이트 80, Tween 80R)을가장많이사용한다.10)
본연구에서는발사탄의 용해도및 생체이용률을증가시 키기 위한 신규고체분산체를 개발하기전에 용해보조제로 사용되는 폴리소르베이트 80이발사탄의 물리적특성에 미
치는영향및피부손상을 검토하고자하였다. 시험 방법
시약및기기
발사탄은 유한약품 (안양, 대한민국)에서 제공받았으며 Tween 20, Tween 80, Span 20 및 Span 80은 산동 DC chemical (서울, 한국)에서 구입한것을 사용하였다. 무모마
우스(Hairless mouse)는 오리엔트 바이오 (서울, 한국)에서
수컷을구입하였다.
발사탄및계면활성제의혼합물의제조
발사탄 1 g을 페트리 접시에넣고 여기에 비이온성 계면
활성제인 Tween 20, Tween 80, Span 20 및 Span 80 0.3 g을각각넣어 3분간교반한 후증류수 0.5 g을넣어수
분을약간함유한 혼합물을제조하였다.11)
혼합물의물리적특성에미치는비이온성계면활성제의영향 각비이온성계면활성제와발사탄과의응집성은육안으로 확인하였다. 또한, 이혼합물을페트리접시에각각넣고증
류수 10 mL를첨가하여 24시간동안방치하여형태의변화
를육안으로 관찰하여분산여부를조사하였다. DSC 측정
발사탄, Tween 80 및혼합물을가지고시차주사열량계
(Differential Scanning Calorimeter, DSC 2010, TA Instruments Inc.)를 이용하여 열역학적 변화를 측정하였다.
이때질소가스의속도조건은 20 mL/min로온도는 0oC에
서 300oC까지분당 10oC씩상승시키면서측정하였다.12,13)
피부손상시험
발사탄, Tween 80 및 혼합물 0.1 g 또는 0.1 mL를 무모
마우스의피부에각각도포하고헝겊으로덮고 24시간방치
한다음희생시키고정상피부 (control)와비교하여 피부발
적및손상정도를육안으로 관찰하였다.14,15)
결과 및 고찰
발사탄및 계면활성제를가지고혼합한 물리적혼합물의 응집성 및 분포성은 Figure 2에 나타내었다. Span 20 및
Span 80의혼합물은반고형상의크림형태를나타내는반면
Tween 20 및 Tween 80의혼합물은 단단한 고상의 형태를
나타내었다 (data not shown). 또한, Span 20 및 Span 80
의 혼합물 (a 및 b)은 물을 첨가하고 24시간 후에 관찰할
경우 서로 응집이 되지 않고 잘 분산되었다. 반면, Tween 20 및 Tween 80의 혼합물 (c 및 d)은 물을 첨가하고 24시
간후에관찰할경우잘분포되지않았으며서로응집이되 어서덩어리로존재하였다. 이런결과들은 Tween 류가 Span
류보다 발사탄에 더 강력한 응집력이 있음을 나타낸다. Tween 류는 HLB 값이약 15로서친수성이어서수용액상에
서 발사탄과 물과의 친화력을 증대시키는 반면 HLB 값이
약 4인 Span류는상대적으로소수성이기 때문에수용액상에
서발사탄과물과의친화력을감소시키는역할을하는것으 로사려된다.16,17)
발사탄, Tween 80 및 혼합물의 DSC 자료는 Figure 3에
나타내었다. 발사탄 (Figure 3b)은약 115oC 부근에서 예리

되어 있다. Tween 80 (Figure 3c)의 경우에는 약 170oC
부근에서 넓은 특정 피크나타낸다. 그러나 혼합물 (Figure
3a)은 발사탄 및 Tween 80의 특정피크가 사라지고 약
150oC 부근에서 새로운 피크가 관찰되었다. 이런 혼합물에
서의물질들의고유특정피크의사라짐과새로운피크의출
현한결과는발사탄이 Tween 80과물리적으로반응하여물
리화학적 상태가변화되었음을나타낸다.18,19)
발사탄과 Tween 80 혼합물의피부손상정도를검토하기
위하여 정상상태 (control), 발사탄, Tween 80 및 혼합물 dmf 무모마우스의피부에각각도포하고 24시간방치후피
부 발적 및 손상 정도를 관찰하였다 (Figure 4). 발사탄 (Figure 4b) 및 Tween 80 (Figure 4c)으로단독으로 처리한
피부는정상상태의피부 (Figure 4a)에비해약간의발적및
손상이 발견되었으나크게 유의할만한변화를 나타내지 않 았다. 그러나혼합물로처리한피부의 경우 (Figure 4d)에는
수컷피부의각질층이심하게훼손되었으며피부발적이현 격하게나타났다. 이런결과는발사탄 및 Tween 80 단독으
로는피부에 손상을 주지않으나 병용투여할 경우에는 서
로 반응하여 심각한 손상을 나타낼 수 있음을 보인다.14,15)
따라서, 추후에 발사탄 및 Tween 80의 반응 정도를 X-ray
및 IR로물리화학적성질을 검토하고병용 투여시위 점막
에대한점막손상정도를심도있게연구할 예정이다.
결 론
발사탄과 Tween 80은혼합시서로 물리적인반응을 하
여응집이발생하고피부에심각한손상을가져오기때문에 발사탄을사용하여제제화하기위해첨가제로폴리소르베이 트류를사용할경우에 커다란주의를요한다.
