A. 인지기능저하의 정의 및 인지기능저하율

V. 결론

연령이 증가함에 따라 인지기능저하 발생의 위험도 증가하며,치매가 발생 한 대상자에서의 인지기능저하는 중요한 보건학적 문제로 인식되고 있다.이 연 구에서는 지역사회 자료를 이용하여 65세 이상 노인의 인지기능저하율을 구하고 노인의 인지기능저하에 영향을 미치는 요인 구명을 통해 인지기능저하 예측모형 을 개발하고자 하였다.

연령,성,교육수준을 보정한 개별 평균 인지기능 곡선은 시간이 지남에 따 라 감소하였으며,성,연령,교육수준별로 동일한 양상을 보였다.또한 남성보다 는 여성,연령이 높을수록,교육수준이 낮은 대상군에서 초기 인지기능 점수가 낮고,인지기능 감소가 더 많이 됨을 알 수 있었다.각각의 인지기능저하방법에 따른 인지기능저하율 7∼17%를 보였다.

인지기능저하 예측 모형은 회귀분석과 의사결정나무분석을 적용해 본 결과 의사결정나무 분석의 결과가 가장 좋은 모형으로 판정되어 의사결정나무분석을 최종 모형으로 선택하였다.모형의 정확도는 90.8%,신뢰도는 69.5%,타당도는 92.5%를 보였으며,기저선의 인지기능점수,가구원수,신체활동,성별,1년 전과 비교한 주관적 기억력 정도가 인지기능저하의 주요 예측인자로 분석되었다.

이 연구는 일개 지역사회 보건소를 방문한 노인을 대상으로 한 것으로 우 리나라 전체 노인으로 일반화하기에는 제한점이 있다.그러나 지역사회 노인 대 상의 인지기능저하 예측모형을 개발했다는 점에서 연구의 의의가 있으며,노인의 인지기능저하 예측을 위한 기초자료로 활용이 가능할 것으로 본다.
