다발성 Sirolimus-eluting stent (SES) fracture에서 경과 관찰 중 추가로 발생한 fracture 1예

관상동맥 중재시술의 발달에 따라 광범위하고 협착이 심한 혈관도 시술이 가능하게 되었다. 이로 인해 스텐트 fracture의 빈도도 늘고 있으며 이로 인한 재협착의 증가 및 시술의 필요성도 점차 보고되고 있다. 특히 우관상동맥은 심장의 움직임에 따른 심한 변화로 fracture가 잘 발생 한다. 저자들은 우관상동맥의 광범위한 협착으로 sirolimus-eluting stent (SES) 2개 겹쳐 삽입한 환자에 대해 경과 관찰로 시행한 관상동맥 조영술 (coronary angiogram, CAG)에서 스텐트 fracture 4군데를 발견하였고, 그 후 재시행한 CAG에서 다시 fracture가 1군데 더 생긴 증례를 경험하여 보고하는 바이다. 67세 여자 환자로 고혈압과 뇌혈관질환을 진단받은 과거력이 있으며, 3년 전 불안정 협심증으로 시행한 CAG에 서 좌전하행지와 우관상동맥의 협착으로 좌전하행지에 SES 3.0ｘ18 mm와 우관상동맥에 SES 3.0ｘ33 mm와 2.75ｘ33 mm를 겹쳐 삽입하 였다. 7개월 뒤 다시 시행한 CAG에서 우관상동맥에 fracture 3군데를 발견하였으나, 증상 및 재협착 소견을 보이지 않아 약물치료만 지속하 기로 하였고, 17개월 뒤 다시 시행한 CAG에서는 총 4군데의 fracture를 발견하였다. 하지만 여전히 증상과 재협착 소견이 없어서 경과 관찰 하기로 하였고, 1년 뒤 시행한 4번째 CAG에서 총 5군데의 스텐트 fracture가 보였고, 근위부 첫번째 fracture 부위에 80% 정도의 재협착이 관찰되었다. 스텐트가 겹친 부위와 재협착된 부위를 포함하여 paclitaxel-eluting stent (PES) 3.0ｘ38 mm를 재삽입 하였고, 현재 환자는 aspirin, clopidogrel, cilostazol의 3자 요법으로 항혈전치료를 받으며 외래에서 추적 관찰 중이다. 다발성 스텐트 fracture가 보고되고 있으나 한 혈관에서 5군데의 fracture가 보고된 예는 없어 이에 증례 보고하는 바이다.

Early heart rate increases during exercise test have influence on parameters of diastolic dysfunction.

Background: Initial response of heart rate (HR) to dynamic exercise has been known as prognostic value of cardiovascular disease. We hypothesized that slow heart rate rise during exercise test was associated with diastolic dysfunction in patients with preserved left ventricular systolic function (EF>50%). Methods: Seventy five patients (35 ± 9 years old, 30 females) who visited our hospital for evaluation of chest pain were included in this study. All patients had negative results of exercise test with modified Bruce protocol. Resting echocardiogram was done with conventional Doppler and tissue Doppler imaging. Early (E) and late (A) transmitral flow velocity and its deceleration time were measured by pulsed-wave Doppler from the apical four chamber view, and medial mitral annular diastolic velocities were measured with tissue Doppler imaging. The medial mitral annular peak systolic velocity, and early (E´)and late (A´) diastolic velocities were determined by spectral tissue Doppler imaging at the medial corner of the mitral annulus from the apical four chamber view using standard methods. Patients with hypertension and/or taking drugs that could influence HR were excluded. Patients with resting hyperactive HR (>100/min) and bradycardia (<50/min) were also excluded. Early HR was calculated by HR at the end of 3 minutes of exercise minus resting HR. Results: Patients were divided into two groups by mean early HR increment (28 beats/min). Early diastolic mitral annular velocity (E´) was higher(13.26±2.7 vs 9.9±2.4, P<0.01) and the ratio of early diastolic mitral inflow velocity (E) to E´(E/E´) was lower (6.1±1.3 vs 7.6 ±1.5, P<0.01) in the group with rapid early HR increment than those in slow early HR increment. Age and other parameters were not different between groups.
Conclusion: Early HR increment was associated with diastolic parameters in patients with preserved LV systolic function.
