예비절개 후 담관삽입 도중 십이지장 천공과 함께 발생한 간문맥 색전증 1례

서론: 내시경적 역행성 췌담관조영술은 췌담관 병변 진단 및 치료에 대한 일차적인 방법으로 사용되고 있으나, 다른 시술에 비해 췌장염, 담관염, 출혈, 천공 등 합병증의 비율이 높다. 그 중 간문맥 공기색전증은 비교적 드물지만 심각한 결과를 초래할 수 있는 합병증 중의 하나로 ERCP도중 내시경적 유두부 절개술 과 같이 유두부 손상을 동반하는 시술을 동반 할 때 주로 발생하는 것으로 알려져 있다. 저자들은 ERCP 도중 예비절개 후 발생한 십이지장 천공과 함께 간문 맥 색전증을 경험하였기 보고하는 바이다. 증례: 63세 여자 환자 내원 2일 전 부터 발생한 우상복부 복통있어 내원하였다. 환자는 특별한 과거력은 없었으며, 혈액검사에서 AST, ALT, ALP, GGT, Bilirubin 상승 소견 보였다. 수술 전 담관 담석의 가능성을 배제하기 위해 내시경적 역행성 췌담관조영술(ERCP)를 시 행하였다. 그러나 총담관으로의 삽관이 용이하지 않아 예비절개(precutting)을 시행한 후 담관 삽입을 시도하였다. 총담관으로 생각되어지는 쪽으로 도관을 삽 입한 후 조영제를 주사하자 관구조물이 관찰되었다가 곧바로 조영제가 사라져 혈관조영을 의심케 하는 소견이 관찰되었고 이어서 복강내부로 조영제가 퍼지 는 소견이 관찰되었다. 이후 예비절개 부위에 작은 천공부위를 확인하였고, 지혈클립을 적용하여 봉합한 후 췌관에 플라스틱 스텐트를 유치하고 시술 마쳤다.
시술 후 환자는 호흡곤란, 복통을 호소하였고, 복부 CT 검사 결과 우측 신장 주변부로 다량의 공기, 간문맥 주위와 우측 간내 간문맥 내에도 상당량의 공기음영 이 관찰되었다. 이에 담즙의 복강 내 누출을 줄이기 위해 경피적 담낭배액술을 시행한 후 비위관 삽입, 금식, 산소 공급, 항생제 투여 등 보존적인 치료를 하면 서 경과를 관찰 하였다. 다음 날 환자 상태는 호전되기 시작하였고 추적검사를 위해 시행한 복부 CT 검사에서 간문맥 주위와 우측 신장 주변부 공기는 감소하 였고, 간내 간문맥 공기음영은 더 이상 관찰되지 않았다. 환자는 내과적인 치료 후 호전되어 퇴원하였고 3주 후 복강경하 담낭절제술을 시행하고 현재 외래에 서 추적검사 중이다. 결론: 간문맥의 공기 색전증은 내시경적 역행성 췌담관조영술 동안 드물게 발생하지만 시술 도중 조기에 진단하기가 어렵기 때문에 심각 한 합병증으로 발전할 수 있는 우발증이다. 또한 시술 후 의식변화, 호흡곤란, 쇼크 등과 같이 환자의 상태가 급격하게 악화될 때 감별 진단을 위해 반드시 고려 해야 한다. 예방을 위해서는 장시간의 무리한 시술을 피하고 시술 도중 또는 시술 후 환자의 상태를 관찰하면서 조기에 합병증을 발견하여 대처하는 것이 중요 하다고 생각된다.

A case of ectopic opening of common bile duct in duodenal bulb

Ectopic opening of common bile duct (CBD) in duodenal bulb is very rare, and it can be missed at endoscopy because of small size of the orifice that lacks typical feature of major duodenal papilla and duodenal deformity that makes endoscopic approach difficult. However, it should not be over- lookedbecause of critical complications such as recurrent duodenal ulcer or cholangitis. Here we report one case of ectopic opening of CBD in duodenal bulb which was managed with endoscopic retrograde biliary drainage (ERBD) through endoscopic retrograde cholangiopancreatography (ERCP). The patient visited us for fever and abdominal pain. There were some significant findings in studies, which included stricture of duodenal bulb with ulcer and one loose slit-like opening in duodenoscopy, in addition to the tapered distal CBD with dilated proximal part in abdominal CT and ERCP. He treat- ed with two times of ERBD insertion which improved duodenal ulcer and cholangitis, and discharged without any biliary related symptom. The com- mon feature of ectopic opening of CBD isknown to be the absence of normal sphincteric structure. Since it allows free movement of bile and duodenal contents between CBD and duodenal bulb without sphincteric control, this disease frequently accompanies recurrent duodenal ulcer and cholangitis that can cause irreversible deformity. Another is the tapered distal CBD (so-called “hook shaped CBD” in several reports) with dilated proximal part in imaging studies. This is assumed to contribute to bile stasis that promotes bacterial growth or lithogeniccharacteristics of bile, which can lead to biliary disease. These complications can be managed with conservative way, but sometimes need surgical procedure, then, these situations need further accu- rate diagnosis because of risk for accidental biliary tract injury during surgery due to abnormal location of bile duct orifice. In conclusion, if we encoun- ter recurrent duodenal ulcer or cholangitis in the field, we have to consider the possibility of ectopic opening of CBD.
