1. 본인은 격리면제서를 발급받음에 있어 격리면제서에 기재된 주의사항을 준수할 것에 동의합니다.
2. 또한 입국 후 공항에서의 진단검사, 자가진단앱 설치, 중앙사고수습 본부의 전화응대와 증상 통보 등 능동감시 의무를 철저히 수행할 것 이며, 만일 격리면제서 발급 신청서에 기재된 내용이 사실과 다르거나 능동감시 의무를 이행하지 않는 경우 감염병의 예방 및 관리에 관한 법률에 따른 형사 처벌에 따를 것에 동의합니다.

2. In addition, I will thoroughly fulfill all my obligations of active monitoring, including COVID-19 testing at the airport upon arrival, installing the self-diagnosis app, answering every call from the Central Disaster Management Headquaters, and reporting symptoms.
I understand that providing false information on the application form for isolation exemption or failing to comply with the obligations of active monitoring will result in criminal punishment pursuant to the Infectious Disease Control and Prevention Act.
