코로나바이러스감염증-19 예방을 위해 위의 사람은 대한민국 입국시 강화된 특별검역절차를 받고 입국 후 14일간 격리를 하여야 함에도 불구하고, 위와 같은 사유로 격리면제를 신청한 바 담당관은 사실관계를 확인하고 대한민국 입국 후 격리면제를 허가합니다.
※ 동 면제서는 해외에서 입국하는 우리 국민과 단기체류 외국인(B1, B2, C1, C3, C4) 및 장기체류 외국인(D8, D9)에게 적용됩니다.
※ 대한민국 입국 시 임시격리시설(1박2일)에서 검사를 받으며, 검사 결과 음성 시 격리가 면제됩니다.
※ 현지에서 격리면제서를 발급받았다 하더라도 진단검사 양성일 경우 치료를 위해 격리됩니다.
※ 입국 후에는 대한민국 검역당국의 안내를 받아 설치한 자가진단앱에 증상 여부를 매일 입력하고 담당 공무원과 매일 통화를 하여 건강상태를 확인받아야 합니다.
※ 동 서식에 기재된 내용이 사실과 다를 경우 감염병의 예방및 관리에 관한 법률에 따라 형사처벌을 받을 수 있습니다.

격리면제서 소지자 입국 안내
□ 각국 주재 대한민국 대사관(영사관)에서 발급된 「격리면제서(Isolation
Exemption Certificate)」를 소지한 내․외국인 입국자(이하 ‘입국자’)는 대한 민국 입국 시 코로나19 예방 및 전파 방지를 위하여 아래의 조치에 협조해야 합니다.
① 입국자는 항공기 출발 전까지 대사관(영사관)에서 격리면제서를 발급 받고, 이를 입국심사대에서 반드시 제시하여야 합니다.
- 입국자는 입국심사대에서 받은 노란색(연두색) 목걸이를 반드시 착용해야 하며, 안내요원의 지시에 따라 선별진료소 또는 임시 생활시설로 이동합니다.
② 입국자는 입국 직후 공항 내 선별진료소(9:00～19:00) 또는 임시생활 시설(19:00～9:00)에서 코로나19 진단검사를 받아야 하며, 그 검사 결과가 나올 때까지 지정된 임시생활시설에서 대기해야 합니다*.
* (대기안내) 입소 시 시설요원의 지시에 따라 입소자 명단 작성, 지정된 대기 장소(방) 내에서 대기 → 검사결과 음성인 경우에만 국내 체류지로 이동 가능
③ 입국자는 보건복지부 「모바일 자가진단앱」을 설치하여야 하며, 입국 다음날부터 14일 간 자신의 건강상태(발열, 기침, 인후통, 호흡
곤란 유무)를 매일 입력해야 합니다.

④ 입국자는 입국 다음날부터 14일 간 능동감시 대상이 됩니다.
- 방역당국 담당자는 매일 1회 이상 전화로 입국자의 건강상태 및
증상 유무를 확인하고 입국자는 이에 성실히 답변해야 합니다.
* 연락처 또는 체류지 변경 시 자가진단앱 콜센터(02-6360-0900)로 반드시 신고
- 특히 입국자가 연속 2일 이상 방역당국 담당자와 전화 연락이 되지 않는 경우 경찰 확인 대상 등이 될 수 있습니다.
□ 만약 입국자가 코로나19 진단검사, 자가진단앱 설치 및 입력, 능동 감시 등 감염병 예방조치에 성실히 응하지 않을 경우 관계법령*에 따라 법적 제재의 대상이 될 수 있습니다.

※ In spite of this certificate, you will be isolated for treatment if tested positive.
※ You should follow the instructions of the health authority to install the Self-diagnosis App on your smartphone and report your health status everyday through the app. You will also receive a phone call on a daily basis from a public official assigned to you to report your health status.
※ Providing false information will result in criminal punishment pursuant to the Infectious Disease Control and Prevention Act.

I understand that providing false information on the application form for isolation exemption or failing to comply with the obligations of active monitoring will result in criminal punishment pursuant to the Infectious Disease Control and Prevention Act.

Information for Travelers with the Isolation Exemption Certificate
□ All travelers (both Korean- and foreign-nationals) entering Korea with the Isolation Exemption Certificate issued by the Embassy (or Consulate) of the Republic of Korea should follow the following instructions when entering Korea, to prevent the spread of COVID-19.
① All travelers shall have the Isolation Exemption Certificate issued by the Embassy or Consulate before the departure of flight and
present the Certificate at the immigration office.
- All travelers shall wear the pass (yellow or green) given at the immigration office around their neck and move to the screening clinic or temporary residential facility following staff instructions.
② All travelers shall be tested for COVID-19 immediately after entry, at a screening clinic in the airport (9:00～19:00) or Living and Treatment Support Center (19:00～9:00). Before the test results are confirmed, they shall wait at a designated temporary residential facility*.
* When entering the facility, travelers shall complete a form following staff instructions and wait in the assigned room. Travelers shall be allowed to move to their residence in Korea only if they are tested negative.
③ Travelers should install the Self-Diagnosis Mobile App (Ministry of Health and Welfare) and record their health status (symptoms such as fever, cough, sore throat, and dyspnea) for 14 days from the day after entry.
