A case of hypothyroidism with rhabdomyolysis, acute renal failure and heart failure

Hypothyroidism have many complication. But heart failure, pericardial effusion, acute renal failure, rhabdomyolysis are rare complication of hypothyroidism. Previously few case was reported about hypothyroidism combined with acute renal failure or heart failure. Initially elderly patient presented with dyspnea, chest discomfort. Finally this patient had severe hypothyroidism with heart failure, pericardial effusion, acute renal failure, rhabdomyolysis. We suspect severe hypothyroidism induced heart failure and acute renal failure, so we start thyroid hormone replacement therapy.
After thyroid hormone replacement theray, patient’s symptoms are improved. Our case suggests that severe hypothyroidism can lead to multiorgan failure like heart failure as well as renal failure.

1Adrenocortical carcinomas are rare and frequently aggressive tumors that may be functional (hormone-secreting) and cause Cushing’s syndrome or virilization, or nonfunctional and present as an abdominal mass or as an incidental finding. We report the case of a 77-year-old woman with cortisol and aldosterone-secreting adrenal carcinoma. The patient complained of general weakness, moon face and weight gain. And she presented with hypokalemia and hypertension. Endocrinological data showed excess of aldosterone production and nonsuppressible cortisol production on low dose dexamethasone suppresion test. Abdominal CT showed a right adrenal mass. She underwent right adrenalectomy and histology revealed the presence of an adrenocortical carcinoma. After adrenalectomy, her hypokalemia returned to normal. The patient is treated with hydrocortisone.
We report this rare case of adrenal carcinoma, sercreting cortisol and aldosteron simultaneously without secreting androgen.
