=Abstract=
Candida is composite genus that is belong to cryptococcaceae, Fungi, Imperfect family.
Candida species is normal inhibitant of the skin, mouth, gastrointestinal track, vagina, bron- chus, and becomes pathogenic in immunosupression frequently.
Candida albicans takes the lead of pathogen of the candida species that brings various clinical syndrome. But the liver abscess caused by candida albicans in very rarely and no report in our country.
1 . 서 론
Candida는 Cryptococcaceae, Fungi, Imperfecti파 (family)에 속하는 복합적 인 종(genus)이 다. 이 들은 청 장안의 업， 위장관， 피부， 칠， 기관지 등에 소수로 상 주하고 있으먹 여러가지 원인으로 면역기능이 억제되 면 주감염 원이 되 기 도 한다. 1) Candida species 중에 서 는 Candida albicans가 주된 병 원성 종。l 며 다양한 임 상증후를 유발시킬 수 있는 것으로 알려져 있다. 그러 나 Candiea albibans에 의 한 간 농양은 매 우 드물고 아 직 국내보고가 없으므루 본 병원에서 경험한 1예를 보 고하는 바이다.
ll. 증 례
환자: 남자， 63세 주소:의상복부동통
헌뱅력 : 환지는 9년천부터 당뇨가 있어서 약물복용
을 해왔었는데 업원 10 일천부터 우상복부에 동동이 았 어서 내원하였다.
과거 력 : 9년천부터 당뇨
이학적 소견 : 혈압， 맥빅， 체온은 정상이 었으며 우 상복부에 압통이 있 었다.
Liver Scan, Sonogram : 간좌엽에 거대한 종괴상 검 사방법 : 간농양내 농을 Transabdominal drainage
로 얻어서 그람염색파 동시에 혈액평판배)1;1 MacConkey
배 치 , thioglycol1ate 액 체 배 지 에 접 종배 양하고 혐 기 성 세균배양을 위하여 혐기성 혈액평판배지에 ;섭종하여
Gaspak jar에서 배 양하였다. 그랍염색상 세균은 보이
지 않았으며 드물게 yeast-like organism을 판찰할 수 있었다. 48시간 혐기성배양에서 칩락。1 보이지 않았으 며 호기성 배양에서는 24시간 후 혈액평판배지에서 작 은 집착이 나타났고 그람염색상 yeast-like organism이 었다. MacConkey배 지 에 서 는 자라지 않았다. Sabou-
raud배지에 계대배양하여 동정하였다. 동정결파는 다 음파 같았다.

API20C에 첩종하여 다음 결파를 얻었다.

결 론칸디다증은 선행칠환。l 있을 해나 항생제요법을 받 았을째또는면역억제제치료를 받았을빼 더잘발 생할 수 있마고 알려져 있다.2) 특히 ]ones3) 등은 당뇨 가 있는 환자에서 정상안에 비해 칸디다증 발생율도 높고 C. albicans의 보균융과 그 밀도가 높았다는 것을 보고하였다. C. albicans블 쥐에 주사하면 그 직후 간 장의 탐식구에서 발견되고 수일동얀 Kupffer세포와 육 아종에서 발견펀다고 한다.4) 그런데 당뇨에서는 파럽 구의 추화능， 탐식능 빛 killing activity가 빨어지는 것으로 알려져 있마. 5) 본 환자는 9년간의 당뇨병력을
가진 사람이 었 다. 그러 나 C. albicans에 의 한 간농양의 보고는 매우 드물냐. Luis 등은6) 98예의 간농양 배 양 결파를 보고하였는데 Candida에 의한 것은 1예도 없었 다. 1979년 Cech7) 등에 의해서 백혈쿠 myelooperoxi - dase 결핍 증파 당뇨가 있는 환자에 서 C. albicans에 의 한 간농양 1예가 보고되어 있다.
C. albicans는 정 상에 서 도 장관내 에 서 소수 존재 하기 때문에 복강내에서 이것이 말견되면 위장판 천공을 강 력히 의심하게 된다.8)-9) 그러나 본 예에서는 위장관 천공은 발견되지 않았다.
N. 고 찰
저자들은 1984년 9월 가툴럭의대 부속 성모뱅원에서 9년간의 당뇨 병력을 가진 환자의 간농양배양에서 Candida albicans를 검 출하였 기 에 보고하는 바이 다.
