Stenotrophomonas maltophilia 복막투석 복막염의 항균제 병용치료 경험 1예

서론: 복막염은 말기신부전 환자 중 복막투석을 시행하는 환자들에서 복막투석을 중단하게 하는 주요 합병증이다. Stenotrophomonas maltophilia는 최근 원 내감염 유발 하는 균주 중 하나로 면역력이 낮은 환자, 유치 도관을 가진 환자에서 감염력이 높으며 다제 내성을 갖는 경우가 많다. 특히 이 균주에 의해 발생한 복막투석 복막염의 경우 아직 그 증례수가 매우 적어 치료를 위한 항생제 요법의 경험이 부족하고 보고된 증례들에서도 치료 예후가 좋지 않아 복막도관을 제 거해야 하는 비율이 높다. 이에 본 저자들은 경구와 복강내 항균제 병용투여로 복막투석 도관 제거 없이 완치된 증례를 경험하여 보고한다. 증례: 53세 남자로 고혈압과 당뇨병이 있었으며 내원1년전부터 말기신부전으로 지속적으로 외래 복막투석을 시행하고 있는 환자로, 내원 전날 전부터 복부통증이 시작되고 복막 투석액이 혼탁하여 입원하였다. 혈압 176/90 mmHg, 맥박 79회/min, 호흡수 22회/분, 체온 36.6℃였다. 복부 전체에 압통과 반발통이 있었으며, 투석도관 출 구부에 발적 등 이상소견은 없었다. 입원 당시 말초혈액 검사상 백혈구 6,920/mm3(중성구 72.3%, 림파구 17.5% 및 단핵구 6.5%), 혈색소 10.2 g/dL, 헤마토 크릿 31.4% 및 혈소판 204,000/mm3였다. 혈청 생화학 검사상 혈중 요소 질소 46.9 md/dL, 크레아티닌 12.8 mg/dL, 알부민 3.4 g/dL, 공복 시 혈당 220 md/dL, 칼슘 8.4 mg/dL 및 인 5.5 mg/dL였다. 혈청 C-반응 단백질 3.66 md/dL (0-0.5 mg/dL)였다. 내원 직 후 시행한 복막투석액 분석에서 백혈구 3456/uL (다형핵 백혈구 72%, 단백구 8%) 이었다. 임상양상과 복막 투석액 분석결과로 복막염을 진단하고 경험적으로 cefazolin 과 amikain 의 복강 내 투여를 시작하 였다. 내원 3일째 복막투석액 분석에서 백혈구 11808/uL(다형핵백혈구 45%)로 상승 소견 보여 기존항생제 투여를 중단하고 vancomycin과 cefepime으로 변 경하여 복강 내로 투여하였다. 내원 5일째, 입원 당시 검사한 복막 투석액 배양검사에서 Levofloxacin과 Trimethoprim/sulfamethoxazole에 감수성이 있는 Stenotrohpomonas malophilia 가 동정 되어Ciprofloxacin 복강 내 투여 와 함께 trimethoprim/sulfamethoxazole 경구 투여를 시작하였다. 이후 복막 투석액 의 백혈구 수가 점차 감소하여 내원 11일째 복막투석액의 백혈구수가 60/uL까지 감소하였으며, 복막 투석액 배양 검사도 음전되었다. Ciprofloxain 복강 내 투여는 총 28일간, trimethoprime/sulfamethoxazole은 경구로 총30일간 복용 후 치료를 종료하였다. 현재 외래 추적 중이며, 1년 이상 복막염의 재발 소견 없 이 복막투석 유지 중에 있다.

Sirolimus Based Immunosuppression in the Renal Transplant Patient with Fibroadenoma

Introduction: The incidence of fibroadenoma in the general population is 7% to 13%. Fibroadenoma is caused by hormone (FSH reduction, estradiol in- crease, prolactin increase) and cytokine(TGF increase, acidic FGF increase). With regard to the kidney transplant(KT) recipient, the increased incidence of benign breast disease in female KT patients receiving cyclosporine was reported. In patients treated with Cyclosporine(CsA). However there was no data on the effect of switch from CNI (CsA or tacrolimus) to sirolimus on the progression of fibroadenoma. So we determined on study the effect of sirolimus on the fibroadenoma. The aim of this study is to assess the reversibility of breast fibroadenomas following switch from CNI to sirolimus among a small cohort of female KT recipients. Methods: This open-label, single-arm study was a 2 part study consisting of a test for the detection of fibroadenoma in female KT recipient (Part one), follow up test for the fibroadenoma patient who will switch to the sirolimus
based immunosuppressant regimen (Part two). Results: Screening of 128 female KT patients at Busan Paik hospital revealed 15 breast fibroadenoma patients of more than BI-RADS category 3. Among 15 patients, 46.7% were taking tacrolimus and 53.3% were taking CsA after KT. and 17.7% are diabetic.
(table 1) Breast US, 1 of 3 patients with BI-RADs category 4A were converted to 3A. And 9 of 12 pa- tients with category 3 were converted to category 2. (table 2) Hormone level was decreased. but it is not effective. (Table 3) Cytokine level were measured, and TGF-beta was statistically decreased, but aFGF was no difference. (Figure) Conclusion: Switch from CNI to sirolimus change the BI-RAD cat- egory of fibroadenoma for 1 year. 9 patients were changed BI-RAD category 3 to category 2. But both suggest benign lesions. So, There is no clinical difference. TGF-beta was decreased.significantly.
TGF-beta is correlated with fibroadenoma. Therefore further studies are needed to determine the un- derlying mechanisms for the association between Switch from CNI to sirolimus and fibroadenoma.
