1. 유전자검사의 목적: 유전정보를 활용한 실종아동등 발견 2. 검사대상물의 처리: 검사 후 즉시 폐기
3. 검사대상물은 본래 목적 외로 이용되거나 타인에게 제공되지 않습니다.
※ 다음 각 항목에 대해 관계자로부터 설명을 들은 후 본인이 충분히 이해하였다고 판단되면

1) [ ] 유전자검사의 이익과 위험에 대하여 관계자로부터 충분한 설명을 들었습니다.
2) [ ] 실종아동등이 보호자를 확인하였을 때, 검사대상자 또는 법정대리인이 요청할 때, 유전자검사일부터 10년이 경과하였을 때에는 유전정보를 폐기합니다.
3) [ ] 유전자검사일부터 10년이 경과한 이후에도 검사대상자 또는 법정대리인이 요청하면 보호자를 확인할 때까지 유전정보의 보존기간을 연장할 수 있습니다.
4) [ ] 경찰, 실종아동전문기관, 국립과학수사연구원은 유전자검사 동의인의 개인정보 보호를 위하여 필요한 조치를 하여야 할 의무가 있습니다.
5) [ ] 유전자검사는 검사대상자 또는 법정대리인의 자발적 의사에 따릅니다.
「실종아동등의 보호 및 지원에 관한 법률」 제11조제4항 및 「실종아동등의 발견 및 유전자검사 등에 관한 규칙」 제9조제1항에 따라 위 검사대상자에 대한 유전자검사에 동의합니다.

1. Purpose of genetic testing : Discovery of missing children using genetic information 2. Disposal of test objects : Immediate disposal after test
3. The result of the test are not for purpose other than the original purpose and are not provided to others.
