COVID-19 INSURANCE PROVIDERS LIST INFO GENERAL INFORMATION
Information needed to conclude the insurance contract: name, surname, date of birth, address of residence, passport series, and contact number.
Main requirement from most Ukrainian insurance companies: foreigners residing in countries from the
"green list" according to the classification of the Ministry of Health of Ukraine, or coming to Ukraine from “green” countries.
Limitations on age: 1-60 years. Other categories discussed and viewed separately in each case due to more risks.
Documents required: photocopies of foreign passport.
Contracts are concluded in electronic form. Application form is filled online.

Insurance type: M+ (Covid-19 +standard medical insurance package).
Included:outpatient,hospital treatment and/or observation expenses up to 5,000 UAH; payment for medicines in sum of 500 UAH; emergency dental care up to 500 UAH.
Repatriation expenses: 3,000 EUR.
Contract concluded in Ukrainian before crossing the border in electronic form.
Documents required: Passport
Insurance policy duration: 30/60/90/180 days with the possibility of extension (conclusion of new contract).
Prices: 180 days – 2150 UAH; 90 days – 1350 UAH.
Sum insured: 30,000 EUR
