우연히 발견된 결절성 규폐증(Nodular silicosis) 1예

Introduction: We describe here a case of nodular silicosis without any exposure history and respiratory symptoms. Case report: A periodic medical checkup done at the public health service revealed multiple lung nodules. We decided to do transthoracic needle biopsy. The highly refractile Needle-like particles in the nodule on a polarized light microscope is observed. Acid fast bacilli stain highlights some bacilli in the Nodule. Discussion;The most patients are diagnosed as silicosis on the basis of characteristic radiographic findings and history of employment in a job associated with exposure to silica-containing dust. But we could diagnose him as neither nodular silicosis nor tuberculosis in our case unless biopsy Results revealed silicosis. Characteristic lung tissue pathology in nodular silicosis consists of fibrotic nodules with concentric
"onion-skinned" arrangement of collagen fibers, central hyalinization, and a cellular peripheral zone, with lightly birefringent particles seen under polarized light. His biopsy result is typical of nodular silicosis. Summary: Even the patient not to remember a suspicious exposure suffers from silicosis. Unless the epidemiologic features of the case make the diagnosis of silicosis certain, lung biopsy may be done to establish the diagnosis and to exclude other diseases.

호흡곤란으로 내원한 환자에서 발견된 성대직상방의 평활근육종

서론: 평활근육종은 모든 연부조직에 발생하는 악성종양중 5~6%를 차지하며 이중에서 3%정도가 두경부에 발생하는 것으로 알려져 있다.
보통 소화기계, 후복막강내,자궁등에 발생하는 것으로 알려져있으며 후두에 발생하는 평활근육종은 매우 드문 악성종양으로 수술적 절제술 이 우선 치료로 알려져 있다. 본 증례는 69세 남자에서 성대직상방에 발생한 평활근육종으로 진단받은 후 수술적 절제술을 시행한 드문 예 로 생각되어 보고한다. 증례: 69세 남자가 내원 3일전부터 점점 악화되는 호흡곤란을 주소로 내원하였다. 내원당시 활력징후는 혈압 150/100 mmHg, 맥박수 분당 157회, 호흡수 30회, 체온 36.2℃였고 말초혈액검사에서 백혈구 18,400/mm³,혈색소 15.3 g/dl,혈소판 289,000/mm³였다.
흉부 일반촬영상 폐렴이나 폐부종등 폐병변은 관찰되지 않았으나, 동맥혈가스검사결과 이산화탄소분압 79.8 mmHg, 산소분압 39.8 mmHg, 산소포화도 50.4% 측정되었고 의식소실변화보여 기관지삽관술 시행하였다. 이후 산소포화도 유지되고 활력징후 안정화되어 기관지삽관제 거하였으나 즉시 호흡부전발생하였고 상기도 기도폐쇄 의심되어 기관지절개술을 시행하였다. 기관지내시경상 왼쪽 성대직상방에 성대를 거 의 막고 있는 괴사성 조직이 관찰되었고 흉부 컴퓨터단층촬영상 왼쪽 성대부위로 조영증강된 병변이 관찰되었다. 기관지내시경상 시행한 조직생검에서 괴사성 만성 염증소견이었으나 후두미세수술로 시행한 점막조직생검에서는 평활근육종으로 판명되었다. 이후 레이저를 이용 한 성대절제술시행 후 환자는 호흡곤란없이 퇴원하였고 현재 외래 추적관찰중이다.
