The Analysis of the Treatment of the Extensive Deep Facial Burn with the Early Excision and Split Thickness Skin Graft from Scalp

Backgrounds: Deep partial thickness and full thickness facial burns often lead to severe cosmetic and functional complications. The face has been considered a restricted site for the skin grafting because of the unsatisfactory cosmetic results. But the problems such as contracture and severe hypertrophic scar in the delayed healing site are more difficult to control than the scars in the grafted area.
Methods: Twenty patients treated for facial burns between Mar. 2003 and Feb. 2004 were evaluated in follow-up. The patients were categorized into the following groups according to depth and management of their facial burn. Early excision and grafting with split thickness skin graft were performed 7∼10 days after burn injury.
Results: The scalp had been used as a donor site in these cases. The scalp has the best texture, thickness, and color match for facial reconstruction, and the availability of a large donor site that is well concealed and heals rapidly.
Conclusions: Early excision and Grafting of the wound appears to give better cosmetic and functional results than delayed excision and spontaneous healing. We concluded that early excision and grafting was recommended as the better treatment in wide and deep facial burn, and the ideal donor site was the scalp.
