Upper Gastrointestinal Bleeding in Major Burn Patients

Department of Surgery, Hangang Sacred Heart Hospital, College of Medicine, Hallym University, Seoul, Korea Backgrounds: In major burns, upper gastrointestinal bleeding is a significant and life threa- tening complication. Many articles have been published about the pathogenesis and risk factors of stress ulceration. But the risk factors are not clearly identified yet and the clinical prognosis is generally dismal. This study was designed to evaluate the incidence, characteristics and risk factors of upper GI bleeding in major burn patients.
Methods: We reviewed all the medical records of patients showed clinically important upper GI bleeding from January 2003 to October 2004.
Results: Twenty one patients presented clinically important upper GI bleeding among 2,675 patients (0.78% Incidence). The mean age was 46.5 years and the average burn size was 41.5%
of TBSA (Total Body Suface Area). There were 7 cases of gastric ulcer, 6 cases of duodenal ulcer and only 1 case showed low esophageal ulcer. But 7 patients were not confirmed the bleeding focus through endoscopic examination, because they were hemodynamically unstable states. Among 21 upper GI bleeding patients, there were 9 acute renal failure (42%), 12 respira- tory insufficiency (57%), 7 hyperbilirubinemia (33%), 15 sepsis (71%) and 6 coagulopathy (28%) cases as associated diseases. The mortality rates along with associate diseases were 100% in ARF, 91% in respiratory insufficiency, 85% in hyperbilirubinemia, 80% in sepsis and 90% in coagulopathy.
Conclusions: In care of major burn patients with the known risk factors such as ARF, Respiratory insufficiency, Hyperbilirubinemia, Sepsis and Coagulopathy, we should keep in mind of the possibility of the upper GI bleeding. And the patients who have such risk factors should be considered adequate prophylatic treatments for upper GI bleeding.
