제5장 고비용 의약품 관련 외국의 제도 현황
제1절 캐나다: New Drug Funding Program(NDFP)
신약 펀딩 프로그램(New Drug Funding Program, NDFP)은 캐나 다 온타리오주의 공공 의약품프로그램(Ontario Public Drug Program, OPDP) 중 하나이다. Cancer Care Ontario에서 운영하며 새로운 항암 제, 종종 매우 비싼 항암제에 대해 비용을 제공한다.
이 프로그램은 1995년 온타리오주의 환자들이 양질의 정맥(IV) 항 암제에 대해 동일하게 접근할 수 있도록 하기 위해 시작되었다. 이전에 는 개별 병원들이 정맥투여되는 항암제에 대한 비용을 각자 지불했기 때문에 병원마다 의약품 접근성이 달랐으며 의약품 사용에 대한 결정 이 항상 의학적 편익과 비용지출 가치(value for money)에 대한 근거 에 기반하여 이루어지지 않았다. 병원의 재정 상황에 따라 지역 환자의 고비용 항암제에 대한 접근성에서 차별이 발생하는 것을 막기 위하여 항암제 비용을 별도로 지원하고자 신약 펀딩 프로그램(NDFP)이 구축 되었다.
2009년 11월 현재 신약 펀딩 프로그램(NDFP)은 온타리오주 정맥투 여 항암제 전체 비용의 75%를 담당하고 있다. 병원에서 나머지 25%에 대한 비용을 지불하는데, 이는 신약 펀딩 프로그램(NDFP)이 운영되기 전에 시판허가 받은 더 오래된 정맥투여 항암제에 대한 비용이다.
항암제에 대한 상세 급여내용은 다음과 같다.

— 신약 펀딩 프로그램(NDFP)은 급여에 대해 평가되고 승인된 새롭 고 비싼 병원-기반 정맥투여 항암제에 대해 재원을 제공한다. 온 타리오주의 Committee to Evaluate Drugs와 Cancer Care Ontario의 연합 하부위원회가 신약의 의학적 편익과 비용-효과성 에 대해 검토하며 Committee to Evaluate Drugs가 보건 및 장 기케어부(Ministry of Health and Long-Term Care)에게 펀딩 승인에 대한 권고사항을 제공한다.
— 승인된 지역사회-기반 의약품은 공공 의약품프로그램(OPDP)의 Ontario Drug Benefit와 Trillium Drug Program에서 재원을 조달한다.
— 신약 펀딩 프로그램(NDFP)은 민간병원에서 투여되는 항암제에 대해서는 재원을 제공하지 않는다.
— 신약 펀딩 프로그램(NDFP)은 개인에 대해 항암제 비용을 급여하 지 않는다. 온타리오주 지역 암센터(Regional Cancer Centres), Princess Margaret Hospital, 80개 이상 지역병원에 재원을 제공 한다.
— 승인된 특정의약품에 대한 적용기준을 만족하는 환자에게 투여된 의약품비용을 급여한다.
2011년 3월 현재 급여하고 있는 의약품 및 급여기준은 <표 5-1>과 같다.
