Salmonella 감염증에 의한 급성 호흡부전 증후군 1예

서론 : Salmonella 감염증은 위장염, 장염, salmonella 균혈증, 골수염이나 농양 같은 국소 감염, 만성 보균상태 등의 다양한 임상 양상으로 나타난다. Salmonella는 그람음성의 호기성 간균으로 경구를 통해 감염되어 혈류를 통해 퍼져나가 전신 증상을 일으키게 된다. Salmonella 감염증의 흉부합병증은 폐렴, 폐농양, 농흉, 각혈, 훙막삼출 등이 있으며 최근에는 항생제 치료에 의하여 흉부합병 증이 많이 감소하였으나 저자들은 salmonella 감염증에 의한 급성 호흡 부전 1예를 경험하였기에 보고하는 바이다. 증례 : 16세 여 자 환자는 수학여행 시에 음식을 먹고 구토, 설사 증상이 있은 후 호전되었다가 2주 뒤에 다시 발열, 두통, 설사, 복부 팽만감 발생하 여 5일간 타병원에서 치료하였으나 호전 되지 않아 본원으로 전원 되었으며 내원 당시 환자는 기침과 호흡곤란을 호소하였다. 과거 력 상 특이한 병력은 없었고 혈압 110/70 mmHg, 맥박 96회/분, 호흡 20회/분, 체온 36.9 ℃이었다. 전반적으로 급성 병색을 보였고 호흡음 청진시에 양측 폐야에서 라음이 들렸으며 복부 검사 상 장음은 정상 이었으나 복부 전반에 걸친 압통을 호소하였고 간종대 나 비장 종대는 관찰되지 않았다. 사지 검사 시 하체 움직임이 저하된 소견을 보였다. 말초혈액검사 소견 상 혈색소 11.9g/dL, 백혈 구 2,900/mm3 이었고, 혈소판 67,000/mm3였다.. 말초혈액 도말 검사 상 밴드모양 다형백혈구가 증가하였으며 혈소판 감소증 소견이 보이고 있었다. 혈청 생화학 검사에서 AST 227U/L, ALT 77U/L, ALP 144IU/L, albumin 3.1g/dL 이었다. 복부 초음파 검사상 간담 도계에 특별한 이상은 없었다. 동맥혈 가스 검사상 pH 7.462, PCO2 24.2mmHg, PO2 29.0mmHg, SO2 60.6%이었다. 흉부 X-선 검사 에서 양측 폐부종이 관찰되었으며 흉부 전산화 단층 촬영 검사상 양측 폐에 유리막양 변화 및 소량의 흉막삼출이 관찰 되었다. 뇌척 수액 검사상 특별한 소견은 없었다. Widal test 상에서 H antige titer가 1:320으로 상승되어 있었으며 혈액 배양검사상 salmonella serotype typhi가 배양 되었다. 환자는 내원하여 40.8℃에 이르는 고열이 있었으며 혈압이 70/40mmHg까지 떨어지고 호흡곤란 심하 여 중환자실에서 기도삽관 및 인공호흡기치료 하였으며 2주간 정맥 항생제 치료 후 2주일간 경구 항생제 복용 후 호전되어 퇴원하 였다.

Sarcoidosis initially presenting as nasal cavitary mass

Sinonasal involvement with sarcoidosis is not common. Unless systemic sarcoidosis was previously diagnosed, sinunasal involvement of sarcoidosis is frequently misdiagnosed with infectious rhinitis or chronic rhinitis. 37-year-old woman, complaining of nasal obstruction and rhinorrhea, visited the hospital. The biopsy of nasal cavitary mass was noncaseating granuloma with necrosis and considered as extrapulmonary tuberculosis. Antituberculosis medication was completed for 9 months. However, she developed a conjunctival mass, neck lymphadenopathies, and multiple intrathoracic lymphadenopathies a year after initial presentation of nasal cavitary mass. Serum angiotensin-converting enzyme was elevated. The biopsy of the conjunctival mass was noncaseating granuloma with necrosis as the biopsy of nasal cavitary mass previously showed. With these findings, she was diagnosed with sarcoidosis. She had shown remission with corticosteroids.
