연령에 따른 근력과 혈당간의 상관관계

서론: 한국은 고령화 사회로 진입하면서 당뇨병과 같은 만성질환이 증가하고 있다. 나이가 들면서 지방이 증가하고 근육양은 감소하는데, 노화에 따른 미토콘드리아의 기능이상이 근육감소와 연관되고 인슐린 저항성을 유발하여 당뇨병 발생을 증가시킬 것으로 추정하고 있다.
본 연구에서 저자들은 연령과 당뇨 유무에 따른 근육 및 근력의 감소 정도와 미토콘드리아의 기능이상과의 연관성을 조사하였다. 방법: 고관 절 수술을 받은 환자들을 대상으로 당뇨병군과 대조군, 청장년군(55세 이하)과 노령군(65세 이상)으로 나누어 신체계측(체성분 분석 포함), 혈액 채취, 대사지표를 측정하였다. 또한 isokinetic tester, 악력계와 파악력계를 이용하여 양측 슬관절신전근, 골곡근의 최대우력과 양측 수 부의 최대악력을 측정하였다. 채취한 근육에서 미토콘드리아를 분리하여 미토콘드리아의 산소소모율을 Oxygen-2k로 측정하여 활성도를 평 가하였다. 결과: 근력은 청장년군에 비해 노령군에서 54% 정도로 낮았으며(p<0.05), 당뇨병이 있는 노령군에서 당뇨병이 없는 노령군 및 청 장년군에 비해 69.3% 및 37.2%로 현격하게 낮았다(모두p<0.05). 당뇨병이 없는 두 군에서 근력 및 체성분에 따른 대사지표를 비교시 혈당, 총콜레스테롤, 당화혈색소가 노령군에서 유의하게 높았으며, 모든 부위의 근력과 혈당 및 당화혈색소간에는 유의한 음의 상관관계를 관찰할 수 있었다(r<-0.7, p<0.05). 노령군에서 전체 지방률 및 복부 지방량은 유의하게 높았으나 제지방량, 근육양은 유의하게 낮았다. 또한 전체 지방률 및 복부 지방량과 당화혈색소는 유의한 양의 상관관계를 보였다(r>0.6, p<0.05). 미토콘드리아의 활성도는 모든 군에서 유의한 차이 가 없었다. 요약: 연령이 증가하면서 근육양 및 근력의 감소가 나타나며 이런 현상은 당뇨병이 있는 경우 더욱 현저함을 관찰할 수 있었다.
이는 노령에 따른 근력의 감소가 혈당과 연관이 있음을 시사한다.

Relationship between missing teeth and health status associated with metabolic syndrome in Korean rural elderly

Missing teeth are result of untreated caries or periodontitis which are most prevalent oral infection disease and reflect one`s oral health status.
Historically, caries or periodontitis were the province of dentists for diagnosis and treatment and commonly thought to have little effect on systemic health status. However, the results of recent studies have shown the relationship of oral and dental infections to chronic cardiovascular disease and other systemic illnesses. The aim of the present study was to investigate the association of missing teeth with health status including BMI, blood pressure, fasting plasma glucose, hemoglobin, total cholesterol, AST level, smoking. We evaluated 3,611 (1,494 male, 2,117 female) elderly over age 60 who were lived in Sung-ju area, Korea. The subjects were divided into 3 groups according to missing teeth.
The Group 1 had missing teeth below 8, the group 2 had 8 to 18 missing teeth, and the group 3 had missing teeth above 18.In women, missing teeth were positive correlated with age, BMI, systolic blood pressure, diastolic blood pressure, fasting plasma glucose and cholesterol.
In multiple regression analysis, age, systolic blood pressure, fasting plasma glucose, cholesterol, and AST respectively were associated with missing teeth in men. In women, age, systolic blood pressure, cholesterol level, fasting plasma glucose, BMI and AST level were significant factor of missing teeth. This study shows that components of metabolic syndrome may be a potential risk factor to dental health, so evaluation of metabolic syndrome is needed in korean rural elderly with poor oral health.
