1. Introduction
1 8 5 6년 V i r c h o w가 처음 정맥성 혈전증에 대해 언급한 이후 Pulmonary embolism(PE)은 미국에 서 매년 5 ~ 1 0 %의 병원 사망률을 보이고 있으며 이 는 9 0 %에서 하지에서 발생한 정맥성 혈전에 의한 것이다. 이러한 사망률은 심근경색, 뇌혈관질환 다 음으로 3번째의 혈관질환에 해당된다.
동양 및 한국인에서는 발생율이 비교적 낮게 보고 되었으나 식생활 습관의 서구화로 점차 발생 빈도가
증가하고1 1 , 3 4 , 3 8 , 3 9 )있으며 근자에 와서는 한국에도 고
관절 전치환술 후 심부 정맥 혈전증(Deep vein thrombosis : DVT)의 발생율이 점차 증가하는 경 향을 보이고 있다. 심부정맥 혈전증은 술 후 사망의 원인이 되는 폐색전증을 유발해 치명적인 결과를 초 래하게 되므로 중요하리라 사료된다.
또한 P E는 임상적 진단도 어려울 뿐만 아니라 5 0 %이상에서 사망까지의 시간이 3 0분 이내이기 때 문에 사실상 치료보다는 예방이 더욱 중요하다 하겠 다. 그러나 최근 연구결과에 따르면 P E으로 사망한 환자 중 5 6 %는 예방요법을 받지 못한 것으로 알려 져 있다20). 따라서 이처럼 D V T의 고위험군을 파 악하고 각각의 개인에 맞는 예방적 치료를 시행하는 것이 인공 고관절 치환술(THR : total hip replacement) 후 발생할 수 있는 치명적 합병증을
줄일 수 있는 방법이 될 것이다.
2. Pathogenesis
대개 multifactorial 하지만 V i r c h o w’s triad로 알려진 혈전 생성의 원인 인자, 즉 Venous stasis, Endothelial damage, Hypercoagulability의 발 생이 THR 중 또는 수술전후에 상당한 연관이 있는 것으로 보고되고 있다3 , 6 , 8 , 3 0 ).
1) Venous stasis
전신 마취나 수술 중 하지의 자세 , 수술 후 국소 적 종창, 수술 후 운동 감소, 비교적 고령의 수술군 등은 정맥 혈류의 감소 및 지연을 초래한다6 , 8 , 5 1 ).
2) Endothelial damage
수술부위 조작 및 고관절을 탈구시킨 자세에서 대 퇴 정맥의 손상(twisting, kinking), 골시멘트 사 용시 열에 의한 손상 등이 해당된다7 , 5 1 , 5 9 ).
3) Hypercoagulability
수술 중 조직손상에 의한 thromboplastin 의 활 성화, antithrombin III의 감소와 혈소판의 활성변

3.Risk factors
1) History of thromboembolism
TE disease의 기왕력으로 치료 경력이 있는 환자 는 8배 발생율이 높다
2) Elective lower extremity surgery
발생빈도가 THR - 51%, TKR - 62%, Hip Fx - 48%7 )로 보인다.
3) 골반, 고관절 & 장관골 골절
정맥 조영술로 진단된 D V T의 발생율 골반골절(Pelvis Fx.) : 61%
대퇴골 골절(Femoral Fx.) : 80%
경골 골절(Tibial Fx.) : 77%2 1 )
고관절 골절 후에 예방 치료를 받지 않은 경우 P E에 의한 사망률은 1 0 %이다5 6 ).
4) Multiple trauma
예방적 치료를 하지 않았을 경우 D V T의 발생율 은 5 0 %이다2 1 ).
5) 연령
D V T의 발생율은 2 0 ~ 8 0세 사이에서 증가한다4 ). 또한 c h i l d r e n에서 D V T은 드물다.
6) 악성 종양( M a l i g n a n c y )
Hypercoagulable state 때문에 risk 증가하며 이는 응고성의 증가와 혈전용해 능력의 감소에 의해 발생하며 또한 항 화학치료는 혈관내피에 손상을 주 어 혈전 발생율이 증가한다.
임상적으로 악성 종양 환자의 DVT 발생율은 2 9 %이다.
7) General surgery
3 0분 이상 전신 마취를 하는 경우 증가하며, 고식 적인 수술 방법보다 내시경을 이용한 수술시 발생율 감소한다
8) 고정 ( I m m o b i l i t y )
사체 생검에서 1주 이하 침상안정을 실시한 군의 t h r o m b o s i s의 발생율은 1 5 %이나 1주 이상 침상안 정을 실시한 군의 t h r o m b o s i s의 발생율은 8 0 %였다.
9) 비만( O b e s i t y )
실제적인 위험도는 불확실하다
10) Oral contraceptives & estrogen treat m e n t
전립선암 환자에서 estrogen treatment는 T E의 발생율을 증가시키지만 여성에서 Estrogen replace- ment treatment는 위험이 증가하지 않는다1 1 , 4 3 ).
4. Incidence
1) 외국의 발생 빈도
① 예방적 항응고제의 사용 없이 THR 시행시 DVT 발 생 율 의 보 고 는 4 0 ~ 6 0 %1 4 , 3 5 , 4 0 , 5 0 , 5 9 ), 4 0 ~ 7 0 %2 4 , 2 5 , 2 8 , 4 7 )이며 이중 Asym PE : 25%, Sym PE : 4~19%2 4 , 2 8 , 2 9 ), Fatal PE : 7%9 , 2 4 , 3 0 ),다른 보 고에 따르면 40`~60% incidence rate를 보이며, 1 5 ~ 2 5 %는 proximal femoral vein에 발생하며, 0 . 5 ` ~ 2 %의 fatal PE이 발생한다2 4 , 4 0 ).
② 효과적인 예방을 한 경우 : 10~20%5 0 )

3) 한국의 발생 빈도
① 예방적 조치를 하지 않은 경우
김,서등3 6 )은 10%, 김3 7 )20%, 유등은 1 6 %
② W a r f a r i n등의 예방약제를 투여한 경우

한국에서도 증가 추세를 보이고 있으나1 2 , 3 4 , 3 6 , 3 9 )서 양인에 비해 훨씬 발생 빈도가 낮다3 4 , 3 6 ). 그러나, 서 양에서 과거에 높은 발생율을 보인 원인은 D V T를 유발 할수 있는 risk factor - 체질적인 원인 뿐만 아니라, 여러 가지 요소-평균수술시간(2-3hr), 출혈 양(1650ml), 수혈양(1144ml), 술 후 운동 및 조기 보행의 결여 등도 중요한 인자로 작용했을 것으로 사료된다.
5. Clinical course & location of DVT
D V T는 발생부위 및 병의 정도와 연관하여 f a t a l PE, Nonfatal symptomatic PE, Asymptomatic PE, Proximal DVT, Distal DVT 그리고 Post-Phlebitic syndrome 등으로 구 분할 수 있으며 D V T와 관련된 임상증상으로 최근 발생한 하지부 동통성 부종, erythema, positive H o m a n’s sign등이 있다. PE이 발생한 경우에는 갑작스런 pleuritic chest pain, shortness of breath, diaphoresis, anxiety등이 나타나지만, 이 들 P E과 관련된 증상들은 비특이적이고, 일단 발생 하면 치명적인 상태를 보이기 때문에 D V T의 예방의 궁극적인 목적은 fatal PE를 막는데 있다. 일반적으 로 proximal DVT는 P E와 상당히 관련 있는 것으 로 보고 되고 있으나2 3 ), distal DVT는 P E와 관련이 적고, 무 증상인 경우가 대부분이며, 저절로 소실되 기 쉽고, 색전증이 발생하는 경우는 드문 것으로 알 려져 있다2 6 , 4 1 ). 그러나 일부의 보고4 4 , 4 8 )에서 d i s t a l t h r o m b u s가 근위부로 전파 될 수 있고, 이는 곧 잠 정적인 P E의 위험요소라고 하여 지금도 이에 대한 계속적인 연구 및 관찰이 이루어지고 있다.
발생시기(peak incidence)에 대해서는 수술후 4
일5 7 ), 7~14일3 5 ), 3주1 6 )등 다양하게 보고되고 있기
때문에, 이에 따른 fatal PE의 가능성을 줄이기 위 해서는 수술 후 적어도 4주까지는 조심해야한다고
하였으나 논란이 많다.
6. Diagnosis
D V T의 조기 진단으로 가장 정확하고 단순한 방 법을 찾는 것이 현재까지 계속되는 관심 중의 하나 로 Screening test가 갖추어야할 조건으로는 근위 부 혈전을 색출할 수 있어야 하고, 환자에게 고통이 없어야 하며, 안전성, 재생가능성 그리고 경제성 등 을 들 수 있다.
1) 임상증상 및 소견(symptom & sign)
부정확하며 높은 의양성 및 의음성을 나타낸다.
특히 P E이 있는 환자의 50% 이상에서 증상이 나타 나지 않을 수 있다. Harris 등3 0 )은 이학적 검사로는 T h r o m b o e m b o l i의 5 0 ~ 9 0 %를 발견할 수 없었다 고 하였고, 광범위한 조사를 통해 확인 한 바 사망한 환자의 단지 2 %에서 사망 전 진단이 가능하였다고 하였다. 이는 P E으로 진단되기 전에 이미 이에 의 해 치명적인 결과를 초래할 수 있다는 것이다.
2) 정맥 조영술( V e n o g r a p h y )
DVT 진단의 gold standard 방법이며, 높은 감 수성과 특이성을 갖고 있다.
양성 소견으로는 constant intraluminal filling defect, invisible of vein, abrupt termination of vessels, collateriation of blood flow등이 있다3 0 ).
3) Doppler ultrasound
정맥내의 혈전에 의한 정맥의 c o m p r e s s i b i l i t y와 f l o w의 변화에 근거한다. 조직의 2차원적 단면을 통 해 직접 혈전을 관찰할 수 있고, 다음과 같은 장점과 단점이 있어 screening, monitoring, manage- m e n t에 있어서 훌륭한 방법이다.
4) 자기공명 영상 혈관 촬영술(MR venography) - 적응증( I n d i c a t i o n )
D V T가 의심되지만 routine screening test 상

5) Nuclear venography
Radio-labeled macroaggregated albumin을 사용한다.
- 장점
정맥 조영술 보다는 덜 불편하고, 과민반응도 적 으며, 동시에 lung perfusion scan도 가능하다.
Proximal thrombi에 대해서는 8 0 ~ 1 0 0 %의 진 단 정확도
- 단점
Distal thrombi에 대해서는 정확도가 적다3 3 )
6) Impedance plethysmography
O c c l u s i o n시 정맥의 f l o w나 volume 반응에 근 거하며, “b l u n t e d”r e s p o n s e는 proximal throm- b u s의 진단에 도움을 준다.
- 장점 비 침습성이고 쉽게 적용이 가능하며
가격이 싸서 screening test로 유용하다.
- 단점
Distal thrombi의 진단이 어렵고, Old thrombi를 구별하기 힘들며,
수술 후 2일 내에는 시행하기 힘든 점이 있다4 9 ).
7) 125-I fibrinogen test
Radioactive marker로 l a b e l e d된 fibrinogen 이 t h r o m b u s에 incorporation 되는 점에 근거한다.
- 장점
방법이 단순하고,
9 0 ~ 9 5 %의 정확도 및 o l d와 new thrombi를 구 별할 수 있다
- 단점
Proximal thrombi의 감별이 어려운점, 이는 수 술 상처부위에 침착 되기 때문이다.
일반적인 골절, 봉와직염 관절염 등에도 침착되기 때문에 의양성이 많다5 4 ).
7. Prophylaxis
1) 시기별

・Hypotensive epidural anesthesia, 70분 이 하의 숙련된 수술 술기는 DVT 발생율을 낮춘다.
・Epidural a는 하지의 blood flow를 증가시키 고 venous stasis를 낮추어 D V T5 8 , 6 2 ) r i s k를 낮춘 다5 , 1 0 ).
・Epidural a & spinal a는 술 후 DVT 발생율 을 50% 낮춘다6 2 ).
③ P o s t o p e r a t i v e

2) 방법상
Ⅰ Mechanical prophylaxis
약물요법에 의한 부작용이 없기 때문에 임상의사 들의 선호도가 있으나 아직 국내에서는 널리 보편화 되지 않은 것도 있다.

㉠ Calf muscle pump2 , 6 3 )
하지의 active movement는 근육의 구획의 압력 을 높이고 이는 정맥을 압박하고 또 정맥의 환류를 높인다. 이에 정맥의 valve system에 의해 혈류의 방향이 근위부를 향하게 한다.
㉡ Venous foot pump4 2 )
plantar venous plexus의 직경은 post. tibial v e i n보다 2개가 크다. 또한 체중 부하를 할 때 plantar arch가 flattening 되면 이에 따라 p l a n-
tar plexus vein의 직경이 작아지고 이에 따라 v e i n에 압박력이 전달되며 v a l v e에 의해 혈류의 방 향이 근위부를 향하게 된다. 이는 근육의 a c t i v e c o n t r a c t u r e와 관계없이 발생한다1 8 , 1 9 ).
현재는 proximal thrombi에는 별 효과가 없고
1 7 ), 일부 distal thrombi에 효과가 있는 것으로 알
려져 있고, 따라서 약물요법의 보조적인 방법으로 추천되고 있다1 5 ).

Ⅱ Chemical prophylaxis (1) Warfarin
(Coumadin : Dupont pharmaceutials) - W a r f a r i n은 os dept에서 가장 흔한 p r o p l y- laxis 제재이다.
-작용기전
・Active vit K-dependent coagulation cas- c a d e를 저해함으로서 anticogulation effect(fac- tor 2, 7, 9, 10, protein c)
・Inactive coagulation factor가 active fac- t o r로 전환을 방해함으로 기존에 만들어진c o a g u l a- tion factor에 대해서는 작용을 못한다. 따라서 첫 투약 후 27-72 hr 정도의 작용시간이 필요하다.
・대개 술 전이나 수술 당일 5~10 mg을 o r a l로 복용한다.
・INR(intermational normalized ratio)을 2 0 - 2 5로 유지
・Total hip 후에는 예방효과가 입증됐지만 T K R 후에는 아직 불명확

(2) Fractionated heparins
LMWH : 분자량이 4000-6000 d(dalton)을 가진 h e p a r i n의 fractionated form으로 a n t i - t h r o m b i c e f f e c t에 비해 bleeding 유발을 감소시킴.
・Antithrombin III와 부착하여 c o m p l e x를 형 성 후 coagulation factor Xa와 I I a를 i n a c t i v a- tion 시킴
・Unfractionated heparin에 비한
・장점 PTT monitoring이 필요 없다.
더 낮은 혈소판 기능 제한 및 이로 인한 더 낮은 출혈성 합병증 발생률,
4배이상의 긴 반감기,
9 0 %이상의 높은 생물학적 이용률, 몸무게에 따라 고정된 용량을 사용
・단점 bleeding (warfarin에 비해 빈도가 높 다)Heparin induced thrombocytopenia
(3) Aspirin
P l a t e l e t의 c y c l o x y g e n a s e를 억제하며 t h r o m- boxane A2 합성을 저해3 2 )
(plt aggregation & vasoconstriction 야기) A s p i r i n은 ischemic heart disease 나 c e r e- brovascular disease에서 효과적으로 알려져 있으 나 total joint replacement 후의 DVT 예방에는 아직 미흡하다고 알려져 있다.
그러나 routine venographic screening &
detected thrombi의 치료와 병합할 경우 s y m p t o- matic & fatal PE을 줄일 수 있다.
8. Treatment
계속되는 혈전 발생의 위험성을 막고, emboli가 생기는 것을 예방하며, 존재하는 혈전의 소멸에 목 적을 두고 있다. 특히 proximal thrombi는 f a t a l 한 P E의 가능성 때문에 일단 진단이 되면 적극적으 로 치료하여야 한다. 그러나 distal thrombi는 자 연적으로 흡수될 가능성이 있기 때문에 적극적으로 치료할 것인가 혹은 자연적 경과에 대한 관찰만을
할 것인가에 대한 논란이 많다4 8 ).
1) 전신 heparinization(aPTT at 2-2.5times control levels)후에 warfarin 유지용법을 3 ~ 6개 월간 시행한다.
2) Vena cava filter : 항혈전 요법이 실패한 경 우나 e m b o l i가 발생한 경우에 시행한다.
9. Conclusion
비록 국내에서 인공고관절 치환술 후 D V T와 P E 의 보고가 많지는 않지만 환자의 특성에 맞게 적절 한 예방적 조치를 취하는 것이 치명적인 합병증의 예방에 도움이 될 것으로 사료된다.
