Upper limit of normal for alanine aminotransferase level in Korean population: Effect of BMI and TG

Background/Purpose: Current upper limit of normal (ULN) for alanine aminotransferase (ALT) level was set at 40 IU/L regardless of age, sex or risk factors for fatty liver diseases (FLD). A recent Italian study suggested that the ULN for ALT should be adjusted to 30 IU/L for men and 19 IU/L for women by eliminating the influences of risk factors for FLD. However, there has been no data on Asian population, especially subjects with normal liver histology. We, therefore, investigated the normal range of ALT level for healthy Korean subjects. Method:
We retrospectively reviewed the medical records of 23, 233 subjects who were examined at Health Promotion Center, Asan Medical Center, from Jan 2005 to Apr 2006. Questionnaires about past medical illness, medication, alcohol, and smoking were taken from all the study subjects. Subjects were excluded from the analysis if they had any of the followings: current medication history, alcohol consumption (>40 g/week), DM, hyperlipidemia or positive results on HBsAg or anti-HCV. The UL was defined as 95th percentile. To validate the UL obtained from ‘presumed' healthy subjects, we compared with that of 1107 healthy liver donors. They were negative for HBsAg and anti-HCV, and showed normal histology on liver biopsy. Results: 1) The ULs for ALT in healthy living liver donors were 35 IU/L in men and 25 IU/L in women. 2) The ULs for ALT in ‘presumed healthy subjects’ were 37 IU/L in men and 26 IU/L in women. 3) Variables related to UL of ALT in healthy liver donors and ‘presumed healthy subjects’ were BMI and Triglyceride. 4) ALT levels increased with age. The upper limits for ALT in subjects over 40 were than higher than those below 40.5) There was a positive correlation between age and BMI.
Conclusion: ALT levels increased with BMI and serum TG level even in normal healthy subjects proven clinically and histologically. To date, our liver-biopsy proven healthy subjects may be the most suitable population in determining ULNs for ALT. So, ULNs for ALT in Asian subjects are 35 IU/L for men and 25 IU/L for women, which are higher than those in Italian study. This discrepancy might result from different age, BMI or serum TG levels.

대장암 근치적 절제술 후 추적 대장 내시경에서 이시 선종 발생의 예측 인자 분석

서론: 대장암으로 근치적 절제술을 시행 받은 환자는 이시 대장암이나 그 전구 병변인 이시 선종을 진단하고 치료하기 위해 추적 대장내시 경검사가 필요하다. 그러나, 아직까지 이러한 이시성 병변 발생 위험도에 영향을 미치는 수술 전 임상적, 내시경적 인자들에 대해서는 밝혀 진 바가 없다. 목적 및 방법: 본 연구의 목적은 대장암으로 근치적 절제술을 시행받은 환자에서 이시성 병변의 발생을 예측할 수 있는 수술 전 임상적, 내시경적 인자들을 밝히는 것이다. 연세의대 신촌세브란스 병원에서 2002년 1월부터 2004년 12월까지 대장암으로 근치적 수술을 시행 받은 총 288명의 환자를 대상으로 하였으며, 진행 선종은 크기가 1 cm 이상, 융모상 혹은 관상-융모상 선종, 고도 형성 이상을 동반한 선종으로 정의하였다. 결과: 환자들의 평균 연령은 59.7±10.0세였으며, 성비는 남:여=1.77:1이었다. 이 중 141명의 환자들은 수술 전 대장내 시경 시행 1년 후에 추적 내시경을 시행하였고, 나머지 147명의 환자들은 2-5년 후에 시행하였다. 1년 후 추적 내시경을 시행한 환자들 중에 서는 47명(33.1%)에서 선종, 6명(4.2%)에서 진행 선종이 발견되었다. 단변량 분석(p<0.05)을 통해서 남성, 흡연력, 동시 선종, 동시 진행 선종 이 이시 선종 발생의 예측인자로 밝혀졌으나 다변량 분석에서는 통계적으로 유의한 인자는 없었다. 수술 2-5년 후 시행한 추적 내시경을 통해 56명(37.8%), 9명(6.1%)의 환자에서 각각 선종과 진행 선종이 발견되었다. 단변량 분석시 이시 선종 발생의 예측 인자로 60세 이상의 연령, 남성, 흡연력, 수술 전 내시경에서 동시 선종, 동시 진행 선종, 다발성 선종이 있었다. 다변량 분석을 시행하였을 때, 흡연력(odds ratio 3.18; 95% CI, 1.10-9.24, p=0.033)과 동시 다발성 선종(odds ratio 2.88; 95%, CI, 1.09-7.59, p=0.033)이 예측인자로 작용하였다. 결론: 본 연구 를 통해 대장암으로 근치적 절제술을 시행 받은 환자들에서 이시 선종 발생을 예측할 수 있는 수술 전 임상적, 내시경적 인자들을 확인할 수 있었다. 연구 결과, 추적 대장내시경 검사를 계획할 때, 흡연력과 수술 전 대장내시경에서 다발성 동시 선종 유무를 고려하여야 하겠다.
