간성 뇌증을 동반한 A형 간염 바이러스와 E형 간염 바이러스의 동시감염 1예

서론: 최근 국내의 위생환경 개선으로 성인에서 현성 급성 A형 간염 발생이 증가하고 있지만 급성 E형 간염의 보고는 드물다. 또한 A형 간염 및 E형 간염 바이러스의 동시감염과 그 임상 경과에 대한 국내 보고는 거의 없는 실정이다. 저자들은 간성 뇌증을 동반한 A형 간염 및 E형 간염 바이러스 동시감염 1예를 보고하는 바이다. 증례: 24세 여자가 3일 동안의 열감을 주소로 개인의원 방문하여 간기능 이상으로 전원되었다. 내원 6주 전에 5일간 태국 여행을 다녀왔으며 가족력, 과거력에서 특이사항 없었고 음주력, 약물 복용력도 없었다. 내원시 혈압 110/70 mmHg, 맥박수 72회/분, 호흡수 20회/분, 체온 38.5˚C였으며 급성 병색을 보였고 경도의 상복부 압통이 있었다. 백혈구 1,200/mm3, 혈소판 105,000/mm3, 알부민 3.5 g/dL, 총빌리루빈 1.9 mg/dL, AST/ALT 5679/4868 IU/L, ALP/GGT 369/159 IU/L, PT 24.9초, IgM anti-HAV 양성이었고 다른 간염 바이러스 표지자는 모두 음성이었으며 복부 초음파검사에서 특이소견은 없었다. 급성 A형 간염으로 보존 치료 중 입원 3일째 총빌리루빈 5.1 mg/dL, PT 28초, AST/ALT 12018/9588 IU/L, 혈청 암모니아 199 μmol/L로 악화되었고 급격한 의식 혼탁과 행동 변화를 보였다. 신경학적 검사와 뇌 전산화단층촬영에서 특이소견 없었으며 간성 뇌증에 대한 치료를 시행하였다. 추가 검사에 서 IgM anti-HEV 양성, IgG anti-HEV 음성이었으며 입원 7일째 의식 및 행동 장애는 정상화되었고, 이후 임상 증상과 검사실 소견이 호전되 어 입원 19일째 퇴원하였다. 퇴원 10주 후 추적검사에서 간기능은 정상화되었으며 IgG anti-HAV, IgG anti-HEV 양성이었고 IgM anti-HEV 는 음전되었다. 결론: 급성 A형 간염은 보존적 치료로 대부분 호전되지만 E형 간염과의 동시감염에서는 심한 임상 경과를 보일 수 있으므로 본 증례와 같이 급성 A형 간염에서 간성 뇌증과 같은 비전형적인 경과를 보일 때에는 동일한 전염경로를 가진 E형 간염 동시감염 여부도 확인해야 할 것으로 생각된다.

The Long-term Treatment Outcome of Autoimmune Hepatitis in Korea

Background/Aims: Autoimmune hepatitis is a chronic inflammatory disease of the liver of unknown etiology. Immunosuppressive therapy with corticosteroid, often in combination with azathioprine can improve clinical, biochemical and histological feature and prolong survival considerably, but the long-term outcome of immunosuppression in Korea is unknown. Methods: We reviewed the medical records of 86 patients diagnosed as having autoimmune hepatitis at Samsung medical center from 1995 to 2008, and assessed the efficacy and safety of immunosuppression and prognosis of autoimmune hepatitis. Results: Under immunosuppressive therapy 72 patients(84%) reached remission after a mean treatment duration of 3.5±6.6 months. Attempts to withdraw medications were made in after a mean treatment duration of 38.4±19.5 months. Thirteen of these patients(54%) relapsed following treatment withdrawal. Of the 86 patients, 6 patients(7%) experienced disease progression and the overall 5-and 10-year progression free survival was 91.2% and 85.5% respectively. Conclusions: The majority of patients with autoimmune hepatitis achieved remission within 4 month after immunosuppressive therapy, but the frequency of relapse was high after treatment withdrawal. Progression free survival in well managed patient was excellent.
