동시성으로 발생한 폐 편평세포암종과 비강의 비호지킨 림프종 1예

서론: 다발성 원발성 종양은 동일 시기 또는 시간 간격을 두고 발생하는 것으로 최근 발견 빈도가 증가하고 있다. 그 중에서도 혈액 종양이 동반된 중복암은 전체 악성 종양의 0.03%, 중복암의 3.34%를 차지하며 혈액종양의 치료 후 속발성 중복암의 빈도가 최근 증가하고 있는 추세이다. 림프종의 치료 후 속발되는 고형암의 보고는 있으나 종양에 대한 화학적 치료 없이 폐암과 동시성으로 발생한 비강 내 림프종의 증례 보고는 그 예가 드물다. 저자들은 지금까지 보고된 바 없는 폐 편평세포암종 환자에서 병발한 비강내 비호지킨림프종 1예를 경험하여 보고하는 바이다. 증례: 3개월 전 폐 편평세포암을 진단받은 57세 남환이 안구 돌출과 안와의 통증을 주소로 내원하였다. 안구 MRI상 우측 사골동과 우측 대뇌반구의 전두엽까지 침범한 5*5.5cm의 종물이 보였으며 종물에 의해 시신경이 밀리는 양상을 나타냈으며 종물주변으로 부종소견이 나타났다. 비강에서 시행한 조직검사상 미만성 대세포형(diffuse large cell type)의 비호지킨림프종이 관찰되었으며, 면역 조직 화학 염색을 통한 통한 면역 표지자 검사에서 CD20에 양성, CD 3, CD 56에는 음성을 보였으며, Ki67에 양성을 보여 미만성 큰 B 세포 림프종으로 진단되었다.

Increased CA-19-9 level in patients with radiographic pulmonary abnormalities

Measurement of carbohydrate antigen 19-9 (CA 19-9) is recommended for patients with pancreatic cancers; nevertheless, an increased CA 19 9 level has also been reported in patients with various benign lung diseases. We aimed to elucidate the pulmonary radiographic abnormalities and laboratory results associated with increased levels of CA 19-9.This study was performed with a case-control design. Cases included every participant in the cancer screening program who had a high CA 19-9 level (>37 U/mL) without a diagnosis of malignancy. Four times as many age- and sex-matched participants with normal CA 19-9 levels were enrolled as controls. Laboratory results and radiographic features on low-dose chest CT were compared. Multiple logistic regression models were constructed to identify predictors for increased CA 19-9.A total of 119 participants with high CA 19-9 levels and 476 controls were included. The pulmonary radiographic abnormalities were more common in participants with high CA 19-9 levels than in controls. A higher erythrocyte sedimentation rate (ESR), higher hemoglobin A1c (HbA1c), and the presence of bronchiectasis, bronchiolitis, emphysema, and interstitial fibrosis were independent factors for increased CA 19-9.Clinicians should cautiously interpret CA 19-9 levels in patients with high ESR and HbA1C in addition to radiographic lung abnormalities.
