혈액투석 환자에서 적혈구생성자극제 사용과 헤모글로빈 변이성

배경: 혈액투석 환자에서 적혈구생성자극제 사용에 따른 헤모글로빈 변이성(variability)이 알려져 있다. 현재 임상에서 사용중인 적혈구생성 자극제의 약력학 및 약물동력학은 서로 다르므로, 그에 따라 헤모글로빈 변이성에 차이가 있을 가능성이 있다. 방법: 2007년 1월부터 12월까 지 한양대학교병원에서 유지혈액투석을 받은 환자 중 급성 병증이 없었던 72명을 대상으로 헤모글로빈 변이성을 조사하였다. 변이성의 지표 로서 헤모글로빈 싸이클링, 헤모글로빈 변동(variance) 및 헤모글로빈 표준편차/평균 비를 각각 구하여 두 군 사이의 차이를 비교하였다. 헤 모글로빈 싸이클링은 진폭 > 1.5 g/dL, 주기의 지속기간 ≥ 2개월인 경우로 정의하였다. 결과: 총 72명 중 53명(73.6%)에서 헤모글로빈 싸이 클링이 관찰되었다. 헤모글로빈 싸이클링의 빈도(평균 + 표준편차)는 1.4 ± 0.9회/년이었고, 진폭은 2.4 ± 1.6 g/dL, 주기의 지속기간은 4.1
± 3.1개월이었다. 적혈구생성자극제의 용량 변경 빈도는 3.7 ± 1.4회/년, 그 중 중단 횟수는 1.5 ± 0.7회/년이었다. 헤모글로빈 싸이클링의 빈도는 Darbepoetin alfa 사용군(n=27)에서 Epoetin beta 사용군(n=27)에 비해 높았고(1.63 ± 0.93 vs. 1.00 ± 0.88회/년, p<0.05), 그 진폭이 컸으며(2.88 ± 1.48 vs. 1.88 ± 1.60 g/dL, p<0.05), 그 속도도 빨랐다(1.21 ± 0.74 vs. 0.73 ± 0.66 g/dL/month, p<0.05). 헤모글로빈 변동은 Darbepoetin alfa 사용군에서 Epoetin beta 사용군에 비해 컸고(1.29 ± 0.70 vs. 0.79 ± 0.53 g/dL, p<0.05), Epoetin alfa 사용군(n=18)에서도 Epoetin beta 사용군에 비해 헤모글로빈 변동이 컸다(1.08 ± 0.52 vs. 0.79 ± 0.53 g/dL, p<0.05). 헤모글로빈 표준편차/평균 비 또한 Darbepoetin alfa 사용군에서 Epoetin beta 사용군에 비해 컸고(10.81 ± 2.10 vs. 8.20 ± 2.59%, p<0.05), Epoetin alfa 사용군도 Epoetin beta 사용군에 비해 컸다(10.30 ± 2.10 vs. 8.20 ± 2.59%, p<0.05). 결론: 혈액투석 환자에서 헤모글로빈 변이성은 현저하며, 사용하는 적혈구생성 자극제에 따라 그 변이성에 차이가 있다.

A Case of Acute Renal Failure in Paroxysmal Nocturnal Hemoglobinuria

Paroxysmal nocturnal hemoglobinuria(PNH) is an acquired chronic hemolytic anemia characterized by persistent intravascular hemolysis subject to recurrent exacerbations. In addition to hemolysis, there is often a risk of venous thrombosis, pancytopenia. Infection, iron deficiency anemia are common, but acute renal failure is rarely developed in PNH patients. A 39 year- old female patient was diagnosed with paroxysmal nocturnal hemoglobinuria 2 years ago, and she had received low dose oral prednisolone. Now she developed gross hematuria, oliguric acute renal failure(ARF) with hemolysis. There was no evidence of renal vein thrombosis. Renal biopsy showed features of acute tubular necrosis(ATN) and hemosiderosis. Magnetic resonance imaging(MRI) also showed characteristic features of renal hemosiderosis. The patient became stabilized with temporary hemodialysis and intravenous fluid, and the renal function fully recovered 3 weeks later. We present here a case of paroxysmal nocturnal hemoglobinuria with developed oliuric acute renal failure(ARF) documented by kidney MRI and renal biopsy.
