A Case Report with Combined Rare cmplications Associated with Hypothyroidism

Background: Clinical symptoms of hypothyroidism are very different depending on the duration and degree of thyroid hormone deficiency and age.

The most typical symptoms are tiredness, lethargy, and weight gain. Pericardial effusion and sudden hearing loss can occasionally occur, and rhabdo- myolysis with an acute renal injury was reported in rare cases. In this study, we report a case of combined rare complications; rhabdomyolysis with an acute renal injury, pericardial effusion, and sudden hearing loss, associated with hypothyroidism, which normalized after levothyroxine replacement.

Case report: 57-year-old men visited with a history of nausea, dizziness, and sudden hearing loss. The patient has been suffering from generalized edema, cold intolerance and weight gain of 2kg over the past year. He diagnosed with hypothyroidism one year ago, but he didn't take levothyroxine therapy, He also took a statin for dyslipidemia (rosuvastatin, 5mg once daily), but there was no change of statin dose. General physical examination re- vealed the puffy face, pitting edema on the pretibial lesion and non-tender diffuse goiter around the neck. In chest X-ray, cardiomegaly was seen (CT ra- tio: 66%), and electrocardiography has shown bradycardia, low QRS voltage. Pericardial effusion was also found on transthoracic echocardiogram.

Creatinine 1.5 (0.7-1.44 mg/dL) and muscle enzyme: CPK 9300 (43-198 IU/L), LDH 1876 (238-422 IU/L), Myoglobulin 636 (28-72 ng/mL) were high- er than normal in the laboratory findings. In thyroid function test, TSH was 120.9 (0.17-4.05 uIU/mL), freeT4 0.099 (0.79-1.86 uIU/mL), T3 75.0(78-182 ng/dL). He diagnosed with rhabdomyolysis with an acute kidney injury secondary to hypothyroidism. We started levothyroxine replace- ment, and all the symptoms were relieved after treatment. Muscle enzyme, creatinine, and thyroid function were normalized after levothyroxine therapy.

Also, pericardial effusion improved at one month after discharge. Discussion: Clinical manifestations of hypothyroidism vary widely. In this case re- port, we should notice that uncommon complications may occur simultaneously when not treating hypothyroidism.

그레이브스병 환자의 갑상선전절제술 후 정상 부갑상선호르몬 상태에서 발생한 저칼슘혈증의 1예

배경: 심한 그레이브스병 환자의 갑상선수술 후 저칼슘혈증은 흔하고 중요한 합병증이다. 통계학적으로 수술 후 6시간에 측정한 부갑상선호르몬이 10.6 mg/dL이하 일 때 저칼슘혈증을 예측할 수 있다고 보고되었으나 수술 전 부갑상선호르몬이 증가했던 환자에서 수술 후 부갑상선호르몬이 정상으로 되었음에 도 불구하고 심각한 저칼슘혈증이 오는 증례는 국내에 보고된 바가 없다. 이에 저자들은 그레이브스병 환자에서 갑상선전절제술 후 정상 부갑상선호르몬 상태 에서 발생한 저 칼슘혈증의 예를 경험하였기에 문헌고찰과 함께 보고하는 바이다. 증례: 8년전 그레이브스병을 진단받은 36세 여자 환자로 덥고 손떨림이 지 속되어 증상 조절 및 갑상선 절제술을 위해 병원에 내원하였다. 혈액검사 상 TSH 0.005 uIU/mL, T4 7.77 ng/mL이상, 항TSH수용체 항체 40IU/L이상, intactPTH는 152.2 pg/mL, 칼슘 9.4 mg/dL 였다. 수술 전 부갑상선의 비대는 없었고 수술 후 조직병리소견에서 부갑상선은 절제되지 않았다. 수술 후 2일째 손,발 저림과 강직 증상 있었고, 혈중 칼슘 6.6 mg/dL, intact PTH 50.7pg/mL이었다. 정맥 칼슘제 투여 및 경구 칼시트리올과 칼슘제 복용 시작8일 후 증상 소 실 및 혈중 칼슘 9.2 mg/dL로 정상화되어 경구 갑상선호르몬제와 칼시트리올, 칼슘 복용 유지하면서 외래 추적관찰 중이다. 고찰: 이번 증례에서 수술 전 비타 민 D의 측정이 되지 않아 저칼슘혈증의 명확한 원인은 밝히기 어려우나 심한 그레이브스병에서 골대사 이상, 부갑상선호르몬과 칼슘대사 이상은 흔히 발생하 며 이에 기전은 명확히 알려진 바가 없다. 수술 후 저칼슘혈증의 발생은 입원기간의 증가등의 경제적 문제뿐만아니라 그 정도가 심한 경우 심각한 문제를 유발 할 수 있으므로, 수술 후 부갑상선호르몬의 수치와 상관없이 저칼슘혈증 발생을 면밀히 살펴보아야 한다.
