심낭 삼출로 심낭 천자술을 받은 국내 환자의 임상 고찰: 단일 심혈관 센터의 경험

심낭 천자술은 심장 눌림증이 의심될 때,혹은 혈액학적으로 안정된 환자에서 진단 혹은 증상 완화를 위해 시행된다. 저자들은 부천 세종병원 심혈관 센터에 2014년 1월부터 2015년 12월 까지 내원한 만 20세 이상의 성인들 중 심낭 삼출로 심낭 천자술을 받은 환자들의 임상 양상을 고찰해 보았다. 심낭 천자술을 받 은 환자는 총 22명으로 남자 10명, 여자 12명이었다. 평균 나이는 68.14±11.90세 였으며 평균 체중은 60.32±11.86kg 였다. 내원당시 혈압은 111.36/70.45 mmHg 였으며,맥박수는 분당 86.45 회였다. 심 초음파 검사 결과 circumferential pericaridal effusion 20건,loculated pericardial effusion 2건이었으며 삼출 액의 양은 Small <0.5cm 인 경우가 2건 ,Moderate 0.5~2.0cm 인 경우가 7건,Large >2.0cm인 경우가 총 13건이었으며 심장 눌림증의 경우는 22건중 3건 이었다. 심낭 삼출의 원인으로 결핵인 경우는 10건으로 가장 많았으며, 악성 종양 5건, 심박동기 삽입에 의한 외상의 경우가 1건이었다. 그 밖에 원인을 알 수 없는 경우는 6건이었다. 심낭 천자술의 중증 합병증으로 우심실 벽 손상이 1건이었으며 입원중 사망 환자는 1건이었다.

Background: The aim of the present study was to evaluate the predictability of cardiovascular risk between indices using body composition and body mass index in general Korea population. Methods: Data from the Korean National Health and Nutrition Examination Survey (KNHANES) 2008?2011 were used for the analyses (n=15,124). Lean and fat mass were measured using dual X-ray absorptiometry. The appendicular lean mass index (ALM/Ht2, kg/m2) and fat mass index (FM/Ht2, kg/m2) were calculated as appendicular lean mass or fat mass over height squared. Participants were divided into 3 tertiles based on their ALM/Ht2, FM/Ht2, or body mass index (BMI) for both sexes: low, middle, and high tertiles. The Framingham Risk Score (FRS) system was used to assess cardiovascular risk. Participants with 10 year risk for coronary heart disease ≥ 20% were defined as high risk.

Results: There were 6561, 6261, and 6562 men and 2854, 2854, and 2854 women in the low, middle, and high tertiles, respectively. The AUROCs of ALM/Ht2, FM/Ht2, or BMI for high risk were 0.579 (95% CI, 0.567-0.591), 0.586 (95% CI, 0.574-0.598), and 0.554 (95% CI, 0.542-0.566) in men and 0.600 (95% CI, 0.590-0.611), 0.647 (95% CI, 0.637-0.657), and 0.671 (95% CI, 0.661-0.681) in women, respectively. Univariate and multivariate logis- tic regression analyses showed that the ALM/Ht2 and BMI tertiles were associated with increased risk of cardiovascular disease in men. The ALM/Ht2, FM/Ht2, and BMI tertiles were associated with increased risk of cardiovascular disease in women. In men, FRS score was highest among participants in low ALM/Ht2 tertile than among those in the other tertiles. FRS score was highest among participants in high BMI tertile than among those in the other BMI tertiles. In women, the association between ALM/Ht2, FM/Ht2, or BMI tertiles and FRS score was weak. Conclusions: The use of body composi- tion indices may help improve the predictability of cardiovascular risk in Korean population.
