CONSENT FOR MEDICAL EXAMINATION AND SUBMISSION OF DIAGNOSIS BEFORE RE-ENTRY
< 아래사항에 동의하지 않는 경우 재입국허가를 받을 수 없으며, 재입국허가 없이 출국 하는 경우 출입국관리법 제37조의2에 따라 외국인등록 말소 처리됨 >
Re-entry permission shall not be issued unless you provide consent to the following terms. Leaving the Republic of Korea (ROK) without re-entry permission will result in cancellation of Alien Registration in accordance with Article 37-2 of the Immigration Act.
재입국자는 현지 출발일 전 2일(휴일 제외) 이내 의료기관에 방문하여 진단서를 발급받아야 합니다.(단, 부득이한 경우 3일(휴일 제외) 이내 진단). 진단을 받지 않았거나 유효한 진단서를 소지하지 않은 경우 탑승이 차단되거나 입국이 거부될 수 있습니다.
(진단서는 국문 또는 영문으로 발급되어야 함 (현지 사정상 부득이한 경우 현지 언어 발급 서 류도 인정하나 국문 또는 영문 번역서를 첨부하여야 함), 공인 현지 의료기관이 발급한 것만 인정.
검사일자 및 검사자 서명이 기재되어야 하며, 발열·기침·오한·두통·호흡곤란·근육통·폐렴 증상 유무 기재 필요)
※ 단, 출입국·외국인청(사무소·출장소)이 발급한 진단면제서 또는 재외공관이 발급한 격리면제서를 소지한 경우에는 진단서 소지·제출 불필요
방역강화 대상 국가에서 입국하여 항공기 탑승 및 입국검역 과정에서 PCR음성확인서를 제출한 사실이 확인되면 진단서 제출을 면제함
All persons who wish to re-enter the ROK shall undergo a medical examination at a local medical institution within 2 days (excluding holidays) prior to the date of departure for the ROK (Yet, the examination within 3 days (excluding holidays) is deemed as valid under unavoidable circumstances) and shall present a written diagnosis to an immigration officer. Failure to undergo the examination or hold a valid diagnosis report will result in re-entry denial.
(Diagnosis reports shall be written in Korean or English(in case of unavoidable circumstances, diagnosis written in local language deemed as valid if Korean or English translation certificate is attached) and issued by an authorized medical institution. It shall include the date of examination and the diagnosis of fever, cough, chills, headache, difficulty in breathing, muscle pain, and pulmonary symptoms and shall be signed by a medical examiner.)
※ Persons who have the Medical Examination Exemption Certificate issued by a Korean Immigration office or the Isolation Exemption Certificate issued by a Korean embassy or consular office are exempted from the obligation to submit a diagnosis report.
Overseas entrants from countries subject to strengthened quarantine measures, if found to have submitted a negative COVID-19 PCR certificate during boarding and quarantine procedures, are exempt from a medical certificate submission for entry into the ROK.
