<안내>
대한민국은 세계적인 코로나바이러스감염증19 대유행에 대응하여 2020년 4월1일 0시(도착기준)부터 모든 입국자에 대해 자가격리 또는 시설격리 조치를 시행하고 있습니다.
- 시설격리 대상 : B-1, B-2, C-1, C-3, C-4, G-1 체류자격을 가진 외국인, H-2, F-4 체류자 격 소지자 중 확진자 비율이 높은 국가 국적자
- 자가격리 대상 : 상기 비자 이외의 체류자격을 가진 외국인과 한국인
<제출대상 및 미제출 시 불이익>
시설격리 대상 외국인은 본 동의서를 작성하여 대한민국 정부에 제출하여야 하고, 이를 제출 하지 않거나 제출된 동의서에 동의하지 않는 항목이 있는 경우 대한민국 정부는 대한민국의
『출입국관리법』,『검역법』 등에 따라 해당 외국인의 입국을 허가하지 않을 수 있습니다.
<시설 격리>
본인은 자가격리면제서를 소지하지 않고 있고 시설격리 대상 외국인으로서 대한민국의 『감염 병의 예방 및 관리에 관한 법률』 제42조, 『검역법』 제16조 등에 따라 입국 후 대한민국 정 부가 별도 지정하는 시설에서 14일 간의 격리조치를 받음에 동의합니다.

<비용 납부>
본인은 대한민국 정부의 안내에 따라 14일 간 시설 격리로 발생하는 식비, 숙박비, 교통비 등 제반비 용을 충당하기 위해 부과되는 비용(1인당 최대 KRW 2,100,000)을 입국 시 전액 납부하는 것에 동의합 니다.

<출국 조치>
본인은 대한민국 정부의 안내에 따라 14일 간의 시설 격리 및 시설 이용에 따른 비용 납부를 적극적으로 이행하며, 이를 이행하지 않을 경우 부과되는 출국명령 등 대한민국 정부의 조치 를 조건 없이 수용하는 것에 동의합니다.

<Note>
In response to the global COVID-19 outbreak, as of 00:00 April 1st, 2020 (arrival time), the Republic of Korea requires all inbound travelers to be subject to self-quarantine or facility quarantine.
- Facility Quarantine : Foreign nationals holding B-1, B-2, C-1, C-3, C-4, or G-1 visa, and H-2, F-4 visa holders who are nationals of high infection rate countries
- Self-Quarantine : Foreign nationals holding visas other than those mentioned above, and Korean nationals
<Subjects of submission and Consequences for non-submission>
Foreign nationals subject to facility quarantine should complete and submit this agreement to the government of the Republic of Korea. If the person fails to submit the agreement or does not agree with all categories below, the government of the Republic of Korea may deny the entry of that foreign national in accordance with the
『Immigration Act』, 『Quarantine Act』, etc.
<Facility Quarantine>
I, as a foreign national without a Self-Isolation Exemption Certificate and being subject to facility quarantine, agree to be quarantined at a facility designated by the government of the Republic of Korea for 14 days after entry, in accordance with Article 42 of the
『Infectious Disease Control and Prevention Act』, Article 16 of the 『Quarantine Act』, etc.

<Expenses>
I, in compliance with the instructions of the government of the Republic of Korea, agree to pay all expenses (maximum 2,100,000 KRW per person) for accommodation, meal, transportation, etc. incurred by facility quarantine for 14 days.

별첨2 탑승객 안내문(국문,영문)
<탑승객 안내문>
1. 2020년 4월 1일 0시(한국시간) 한국 입국자는 코로나 19의 감염 예방을 위해 검역법 등 관련 법령에 근거하여 원칙적으로 입국일 다음날로부터 14일간 격리 대상자입니다.
2. 한국대사관으로부터 자가격리면제서를 받지 않은 단기 체류 외국인은 한국 정부가 지정한 격리시설에서 격리 생활을 해야 합니다 .
3. 격리시설은 임의로 배정되며, 개인의 사정에 따라 변경은 불가합니다.
공항에서 격리시설까지 이동에는 별도 차량이 지원됩니다 .
4. 격리시설 이용료는 1일 15만원 기준 총 210만원이며, 시설입소 전까지 사전 납부하셔야 합니다 .
5. 이용료는 숙박비, 식비 등이 포함되어 있습니다.
6. 격리시설 내부는 금연구역이며, 격리기간 동안 외부 출입 및 다른 입소자와의 접촉을 금지 합니다 .
7. 격리 기간 중 발열, 기침, 호흡곤란 등 코로나19 의심 증상이 생길 경우 코로나 19진단검사가 제공됩니다. 다만, 음성판정이 나오더라도 14일 경과 후 퇴소 가능합니다 .
8. 한국행 항공편 탑승권을 발급 받으시려는 경우 시설격리 동의서를 작성하여 제출하여야 하며 , 동의하지 않는 경우 한국 입국이 거부됩니다.
9. 입국후에도 위의 사항을 준수하지 않는 경우 검역법 등 관련 법령 에 따른 격리조치 위반으로 간주하여 출국 조치 될 수 있습니다 .
※ 외국인 대상 코로나19 진단검사 결과는 선원교대 목적의 선사 제출
등에 한하여 제한적으로 발급 가능하며 , 입소 후 14일간 격리 기간
중 중도 출국할 수 없습니다 .
<Notice for Travelers>
1. To prevent transmission of COVID-19, travelers entering Korea after 00:00, April 1st, 2020 (Korea time) are subject to quarantine for 14 days from the day after entry in accordance with relevant laws including the Quarantine Act.
2. Short-term travelers (foreign-nationals) without a Self-Isolation Exemption Certificate issued by the Embassy of Korea are subject to quarantine at a facility designated by the Korean government.
3. The quarantine facility is allocated randomly and cannot be changed for personal reasons. Transportation from the airport to the facility will be provided.
4. The total expense for facility quarantine is KRW 2,100,000 (KRW 150,000 daily). The expense should be paid before entering the facility.
5. The expense includes costs for accommodation, meals, etc.
6. The facility is a smoking-free area. You shall not exit the facility nor make any contact with another resident during your stay.
7. If you develop fever, cough, dyspnea, or any symptoms that may indicate infection, you will be tested for COVID-19. Even if you are tested negative, you shall not be discharged before the end of the quarantine period (14 days).
8. If you plan to get a boarding pass for a flight to Korea, you shall complete and submit the Agreement to Facility Quarantine. If you do not agree, your entry to Korea will be denied.
9. Failure of compliance with the guidelines above may be deemed as violation of relevant regulations including the Quarantine Act and result in an order for departure from Korea.
※ COVID-19 test results for foreign nationals may be issued in specific cases, including when they are required to submit the result to shipping companies for sea crew change; it is prohibited for those under quarantine from departing Korea while being under the 14-days quarantine.
