Background/Aims: The prevalence of Non-alcoholic fatty liver disease (NAFLD) is increased in diabetes. Postmenopause is associated with risk of NAFLD. Reproductive factors have been implicated in many health consequences for women in later life. The objective was to determine whether there was an association the reproductive factors and NAFLD in postmenopausal diabetic women.
Methods: This study was based on the data from the Korean National Health and Nutrition Examination Survey (KNHANES), conducted by the Korean Ministry of Health and Welfare in 2013-2016. This survey is a cross-sectional and nationally representative study of noninstitutionalized civiians using a stratified, multistage, clustered probability sampling design. Of 31,098 participants, data for 4,102 postmenopausal women were included in the analysis.
Results: The The prevalence of NAFLD was 55.4% in postmenopausal diabetic women. After fully adjusting for confounding factors, including lifestyle, sociodemographic factors, metabolic factors and duration of diabetes, age at first childbirth ≥ 30 years (odds ratio 0.390 (95% CI 0.225-0.677)) and age at menarch (OR 0.678 (0.477-0.964)) were significantly associated with NAFLD. Other reproductive factors including age at menopause, number of pregnancies, oral contraceptive use, and lactation history were not associated with a risk of NAFLD.
Conclusions: Later pregnancy and later menarch were independently associated with a lower risk of NAFLD in postmenopausal women with diabetes.
