○ Seamen visa(C-3-11)
□ Passport with validity of minimum 6 months
□ One passport sized-photo with white background _Only valid taken within 6 months □ Visa application form
□ Invitation letter from Korea
□ Certificate of business registration from Inviter _Should be issued within 3 months □ Employment contract
□ Seaman’s pocketbook(Frist and last page) _Colored with clear copy □ Last 2 years IT return
(If not eligible -> Last 6 months bank statement with a letter of reason) □ Covering letter
□ Nationality certificate of registry
(If the ship is under construction, submit evidence document instead) □ Guarantee letter from Korea
□ The entry declaration/card of seaman _By ministry of Justice, Korea
□ Memorandum to prevent unauthorized departure and guarantee departure of seaman □ Notification of port entry and departure _Scheduled notice
□ Certificate of registration for shipping business _Korea □ Shipping company registration _India
□ Confirmed flight ticket □ Consent to quarantine
※ The bank statement must submit the original copy with the seal and signature.
※ Submit all the documents received from Korea in original or colored with clear copy. (Invitation letter, etc.)
※ Application with incomplete documents cannot be submitted.
※ Check all documents above and fill the boxes out
※ Correction pen or tape should not be used on the documents.
※ Documents downloaded from the website of the Korean Consulate in Chennai are only be accepted.

※ In terms of the validity of all documents, unless specific validity is mentioned, all documents are only valid for three months since its issuance.
