Purpose: The optimal initial treatment for burn wound is gen-
erally applying cold running tap water of temperature 12∼18oC for approximately 20 minutes. But most of the patients are not susceptible to this initial treatment because they are likely to get embarrassed in such situation. According to sta- tistics of our hospital, 92.2% of patients who visited ER (emergency room) had less than 10 minutes of water cooling.
In this study, our aim was to find out the clinical effect of bio- cellulose sheet (Bestian MⓇ pack) as an emergent treatment.
Methods: Between November 2010 and October 2011, 93
patients with burn wound showing first or superficial second degree at first inspection were evaluated in our study.Biocellulose sheet (Bestian MⓇ pack) was applied at the wound as soon as possible for 25 minutes and second in- spection was done. Routine moisture dressing was done thereafter and patients were followed as outpatient.
Results: Among 93 patients, 28 were male and 65 were
female. Scalding burn, contact burn, flame burn and other causes were 78, 6, 4 and 5 patients respectively. Average complete healing time was 7.99 days and average follow-up time was 5.49.Conclusion: Biocellulose sheet has the effect of cooling
down the skin temperature up to 6∼8oC after 10 minutes of its application. Recent studies reported that appropriate cool- ing in burn wound improves the zone of stasis and zone of er- ythema histologically eventually reducing the chance of skin graft. Therefore, biocellulose sheet has the effect of improv- ing initial pain, histologic status, healing time and cost in pa-tients with burn wound whose depth is not deep. Furthermore it reduces the risk of scar formation. (J Korean Burn Soc

일반적인 화상의 응급처치는 흐르는 수돗물에 20분 정도 식혀주는 것이다. 이러한 간단한 응급처치로도 화상의 깊 이를 줄여주고 치료기간을 단축시킬 수 있다
1,2). 하지만 많 은 환자들이 사고 당시 적절한 응급조치를 하지 못한 채로 병원을 방문하고 있었다. 본원의 자료에 따르면, 얼음물이 나 수돗물을 차게 틀어 응급처치를 한 경우가 43.1%였고, 응급처치를 하지 않고 바로 인근병원으로 방문하는 경우가 45.4%였다. 소주, 치약을 바르는 부적절한 응급조치를 시행 한 경우가 4.7%였다. 찬물로 응급처치를 한 환자들 중에서 10분 이하의 응급조치 시간이 92.2%로 가장 많았고, 20분 이상인 경우는 3.1%였다(Fig. 1).
본 연구에서는 적절한 응급처치를 못하고 내원한, 초진 소견상 2도 이하로 판단되는 환자들에게 응급처치의 일환 으로 biocellulose sheet (Bestian-M
Ⓡpack)를 사용한 후 그 효용성에 대해 알아보고자 하였다.
대상 및 방법
1. 재료
Bestian-M
Ⓡpack은 biocellulose sheet에 aloe-vera를 첨
가하여 만든 것이다(Fig. 2). Biocellulose sheet는 필리핀 등
지에서 디저트로 이용되고 있는 '나타 데 코코(nata de co-
co)'를 응용하여 만들었는데 99%의 수분과 1%의 100나노
미터의 굵기의 섬유로 구성되어 있다. 고밀도 구조로 피부
부착력과 탄력성이 뛰어나며 다양한 형태로 변형이 가능하
다
3). 의학적으로는 조직공학의 scaffold, 인공혈관 등으로

쓰이고 비의학적으로는 다이어트 음식이나 고강도 종이, 이어폰 등으로 쓰인다. 향후 지속적으로 다양한 분야에 응 용될 것으로 판단된다.
2. 대상
2010년 11월부터 20011년 10월까지 본원 응급실에 내원 한 화상 환자 중 초진 소견이 1도 및 표재성 2도 이하이면서 20분 이내로 응급처치 하여 충분한 처치를 하지 못했다고 판단되는 환자들을 대상으로 하였다. 대상 인원은 93명이 었으며 셀룰로스나 알로에에 과민반응이 있는 환자나 당뇨 병 등의 만성질환이 있는 환자는 제외 하였다.
3. 방법
응급실 내원 즉시 초기소견을 확인한 후 biocellulose sheet (Bestian-M
Ⓡpack)를 부착하였다(Fig. 3). 25분간 부착 후 떼어내 상태를 확인한 후 일반적인 습윤 드레싱을 하였 다. 모든 환자들을 외래 추적관찰 하였다.
결 과
총 93명의 환자 중 남자는 28명, 여자는 65명이었다. 화상 의 원인 별로 보면 열탕화상이 78명, 접촉화상이 6명, 화염 화상이 4명, 기타 원인이 5명이었다. 상처가 회복되는 기간 은 평균 7.99일이었고 평균 외래 방문 횟수는 5.49회이었다.

환자에서 통증의 감소와 발적의 감소를 관찰할 수 있었다 (Fig. 4, 5).
고 찰
여러 연구에서 가장 적절한 응급처치는 12∼18
oC 정도의 흐르는 수돗물을 20분간 상처에 흘려주어 환부를 씻어주는 것으로 보고되고 있다
4,5). 적절한 냉각은 조직학적으로도 zone of stasis 및 zone of erythema를 호전시켜 피부이식을 하는 경우도 줄일 수 있고 2주 후 재상피화 상태나 6주 후 흉터의 상태 면으로도 좋다는 연구가 있다
5,6). Nguyen 등의 연구에 의하면 적절한 응급처치로 상처가 깊어지는 것을 줄여주며 이에 따라 32% 정도의 피부이식 수술의 감소효과 를 보였다고 한다

그 밖의 연구를 보면 Hydrogel을 이용하여 응급조치를 한 경우 수돗물보다는 못하지만 안 하는 것보다는 낫고

Venter 및 Cuttle 등은 10분 이하로 응급처치를 했거나 심지 어 한 시간 이상 지체된 상태에서 수돗물로 냉각시킨다 해도 아무런 조치를 하지 않은 것보다는 낫다는 보고가 있다
4,5). 적절한 냉각을 하지 못한 상태에서 병원에 오는 경우 극 심한 통증과 발적, 발열이 관찰되는데 기존의 항생제 연고 나 기타 습윤드레싱만으로는 환부에서 느껴지는 열감, 통 증을 해결하기 힘들다.
저자들의 통계상 얼음물에 환부를 담그고 병원에 오거나 치약, 소주, 참기름 등의 민간요법으로 응급처치를 하고 오 는 경우도 있었고 심지어 아무런 응급조치도 하지 않고 오 는 경우도 많았는데 얼음물에 담근 경우는 조직괴사의 가 능성이 더 많다는 연구들이 있어
9)응급처치 전반에 대한 교육 및 계몽이 필요하다고 판단된다.
한편, 본 연구에서 사용된 biocellulose sheet (Bestian-M

aloe-vera를 첨가하여 만든 것인데, 고밀도 구조로 피부 부 착력과 탄력성이 뛰어나며 다양한 형태로 변형이 가능하 다. 따라서 신체 어느 부위에 부착하여도 들뜨는 곳이 없이 고르게 부착되고 시원한 느낌을 주어서 피부 친화적이다.
Biocellulose sheet가 나노(nano)구조여서 균의 침입도 어렵 게 되어있고 건조된 형태일 경우엔 흡착력도 높아 이상적 인 상처치료물질이 될 수도 있다

화상부위에 응급처치 후 1시간 뒤 각각의 응급 처치 별 피부온도를 비교하였을 때 처치하지 않은 그룹은 ＋0.44
oC 가 되었고(올라갔고) 물을 사용한 그룹은 −7.82
oC (감소), Hydrogel을 사용한 그룹은 −3.87
oC (감소), 30분이 지연된 상태에서 hydrogel을 사용한 그룹은 −2.67
oC (감소)의 소 견을 보였다는 보고가 있다
8). 본원 연구실의 개발 과정 중 에도 biocellulose sheet (Bestian-M
Ⓡpack)를 부착 후 10분 정도 지나면 피부의 온도가 6∼8
oC정도 내려가고 통증도 감소됨을 알 수 있었다(Fig. 6). 연구실의 결과와 마찬가지 로 대상 환자에서도 Bestian-M
Ⓡpack을 사용하여 통증의 감소는 물론 발적의 감소를 관찰할 수 있었다.
결 론
표재성 2도 이하의 심하지 않은 화상 환자에게 bio- cellulose sheet (Bestian-M
Ⓡpack)을 사용하는 것은 초기의 극심한 통증을 감소시키고 환부의 조직학적 상태를 경감하 여 치료기간 및 비용을 단축시키는 효과를 거둘 수 있을 것으로 판단된다. 나아가 흉터 형성에 대한 위험성의 감소 에도 도움이 될 것으로 판단된다.
