Recent Surgical Treatment of Moyamoya Disease

he authors reviewed the recent trend of surgical treatment for pediatric as well as adult onset moyamoya disease(MMD). Combined direct and indirect arterial anastomosis or multiple indirect arterial bypasss has been increasing for pediatric MMD and direct arterial bypass is recommended for adult MMD, especially in patients with hemorrhagic MMD.
Besides perioperative complications related to the management of MMD and 1999 annual report by the Research Committee on Spontaneous Occlusion of the Circle of Willis(Moyamoya Disease) of the Ministry of Health and Welfare, Japan will be summarizd.

서 론
MMD의 외과적 치료는 크게 직접적인 혈관 문합술과 간 접적인 혈관 문합술로 나눌수 있으며 전자의 대표적인 수 술법으로는 superficial temporal artery-middle cerebral artery(STA-MCA)
2)12)14)가 있고 후자에 속하는 수술 법으로는 encephaloduro-arteriosynangiosis(EDAS) 혹은 encephalomyosynangiosis(EMS)
1)13)15)22)25)등이 있다.
본 논문에서는 소아와 어른의 MMD에서의 최근의 수술 방법의 경향, 특히 출혈성 MMD에 대한 치료동향, MMD에 서의 수술과 관련된 합병증과 최근 일본에서 발간된 1999 년 MMD에 대한 연보등을 정리해 보고자 한다.
소아의 모야모야병
소아의 MMD는 일반적으로 성인에서 보다는 빨리 진행 되는 경향이 있으며 특히 2세 이하에서는 그 예후까지도 나쁜 것으로 알려져 있다.
그러기에 소아의 MMD에서는 특히 조기 진단과 조기 치 료가 어른에 비하여 더 절실한 것이 사실이다.
최근에 보고된 소아의 MMD에 대한 외과적 수술방법을 고찰해 보면, STA-MCA 문합술과 같은 직접적인 수술법 이 소아에서는 혈관직경이 작아서 수술이 어렵고, 수술시 간이 길며, 뇌표면의 혈관을 일시적이나마 차단해야 된다 는 단점이 있는 반면, MMD에서 흔히 볼수있는 뇌허혈에 대하여 많은 뇌혈류를 즉각적으로 공급 할 수 있으며 간접 적인 수술법이 실패된 경우에 활용할 수 있는 장점 때문에 STA-MCA 문합술을 시행해야 하는 경우
2)4)12)14)들이 있 지만, 그러나 많은 경우에서 간접적인 수술방법을 선호하 고 있는 경향이다. 그 이유로는 MMD의 자연적인 진행과

정으로 인하여 MCA의 원위부까지도 다발성 협착 내지는 폐색이 초래되어 STA-MCA 문합 상태가 계속 기능을 유 지하지 못한다
1)2)11)고 하는 보고와 전술한 이 수술의 단점 때문이다.
최근에는 MMD에서 혈관문합술을 시행함에 있어서도 가 능하면 광범위한 뇌표면에 여러가지 방법을 병합하여 수술 함으로써 더 좋은 결과를 얻고있다

8)이 일본에서 소아 MMD를 위하여 최근에 시행되고 있는 치료내용을 보면 20%에서는 수술하지 않고, 30%에서는 한 가지의 간접적인 혈관문합술만 시행하고 있 고, 20%에서는 직접 및 간접 혈관문합술의 병합술을, 18%
에서는 여러가지의 간접적인 혈관문합술을 동시에 시행하 고 있었다.
그러나 Houkin 등이 지난 12년간 encephalo-duro- arterio-myo-synangiosis(EDAMS)와 STA-MCA 문 합술을 병합하여 시행한 34측의 소아 MMD와 22측의 어 른 MMD에서 수술후 감산혈관조영술(digital subtraction angiography) 결과를 보면, deep temporal 동맥과 mid- dle meningeal 동맥이 소아에서는 좋은 신생혈관 형성에 크게 기여한 반면, STA는 소아나 어른에서 모두 별로 효 과가 없었으며, 단순한 EDAS가 항상 효과적이진 않았다
6). 고로 측두근과 뇌경막을 동시에 이용하는 간접적인 혈관재 건수술이 소아에선 도움이 된다고 볼수 있다.
최근에는 수술의 적응증도 positron emission tomogra- phy(PET)나 single positron emission computed tomo- graphy(SPECT)를 통한 뇌혈류 예비량을 기준으로 하고 있고, 수술후에도 비침습적이며 전신 마취를 피할 수 있는 magnetic resonance angiography(MRA)를 통한 추적검 사가 보편화되고 있다.
성인의 모야모야병
Kobayashi 등
19)은 42명의 출혈성 MMD 환자를 bypass 수술 없이 평균 80.6개월 동안 추적조사 하였던 바 처음 출혈 후 10년 이상된 환자 5명(35.7%)에서 재출혈이 발 생하였고, 재출혈율은 7.09%/person/year로 나타나서 MMD 환자의 예후에 매우 나쁜 인자인 재출혈이 적지않음을 보 고하였다. 더군다나 재출혈시에 3명(21.4%)에선 출혈부위 의 뇌반구가 바뀌었고 7명(50%)에선 뇌출혈의 형태도 바 뀐 점으로 보아 재출혈의 예방이 쉽지 않음을 시사하였다.
5명의 성인 MMD에서 STA-MCA 문합술 전후에 시행 한 PET 검사에서 수술후에 중대뇌동맥영역의“misery”
관류가 개선되고 기저핵(basal ganglia) 부위의 뇌혈류량
이 감소된 소견은 수술후 뇌동맥조영상에 나타난 모야모야 혈관의 감소와 일치된다고 볼수 있다

Okada 등
27)은 성인 MMD에서 STA-MCA 문합술이 뇌 순환 개선과 모야모야혈관 감소에 도움이 되나 재출혈을 완 벽하게 예방하진 못한다고 하였다.
일본의 여러병원이 참여한 조사에서는 출혈성 MMD 환 자중에서 bypass 수술을 받은 152명중 29명(19.1%)이 재출혈을 보인 반면, 보존치료를 받은 환자에서는 138명중 39명(28.3%)이 재출혈을 보여서, 수술이 재출혈의 빈도를 줄이는 효과가 있었다

Yoshida 등
33)이 28명의 출혈성 MMD환자를 평균 14.2%
년 추적하였던 바, 5명은 일차 출혈에서 사망하고, 남은 21 명중 6명에서 일차 출혈후 2년~20년 사이에 재출혈하여 그중 4명이 사망하였다. 재출혈 환자를 분석하였던 바 수 술받은 8명중 1명과 수술받지 않은 환자 13명중 5명에서 재출혈 하여 수술을 받게 되면 재출혈이 훨씬 줄어들 수 있지만, 통계적으로 의미있는 차이를 보이진 않았다.
Houkin 등
6)은 소아와 성인에서 시행한 STA-MCA 문 합술의 결과를 보면 혈관개통율에 있어 소아에선 28측 중 에서 15측(53%), 성인에서는 18측 중 17측(94%)을 보 이고 있어, 성인에서는 STA-MCA 직접문합술이 매우 유 효함을 알 수 있다.
Kim 등
20)은 MMD 환자에서 광범위한 범위에 직접 및 간접적인 혈관문합술을 병합하기 위하여 전두두정후두부에 걸쳐 STA-MCA, EMS, EDAS와 EDAMS를 병합하여 좋은 결과를 얻었다.
그러나 최근 Kawaguchi 등
17)이 출혈성 MMD를 가졌던 성인 환자 22명을 평균 8년간 추적한 결과 STA-MCA 문 합술을 받은 6명에선 어떠한 종류의 뇌졸중도 일어나지 않 았던 반면에, 보존적인 치료를 받은 11명 중 6명과 EDAS 수술을 받은 5명중 3명에서 재출혈이나 허혈성 뇌졸중이 나타났으며 통계적으로도 STA-MCA 문합술이 의미있게 좋은 결과를 가져 왔다고 보고하였다.
이런 점에서 bypass 수술 자체가 혹은 어떤 종류의 수 술이 재출혈의 예방에 도움이 되는지를 알아보기 위하여는 앞으로 더 많은 출혈성 MMD에 대한 장기적인 조사가 필 요할 것으로 보지만, 최근의 보고들에 의하면 성인에서 흔 하게 발견되는 출혈성 MMD에는 STA-MCA와 같은 직접 적인 혈관문합술이 재출혈 예방에 도움이 됨을 알 수 있다.
수술 합병증
MMD의 수술과 관련된 합병증으로는 뇌졸중

뇌경막하수종 및 혈종
5), 과관류증후군
32), 창상감염 등을 들 수 있으며, 가장 문제가 되는 것은 수술중 혹은 수술 후에 발생되는 뇌졸중이다.
MMD 수술후 보고되는 뇌졸중은 3.8%에서 22.2% 까지 보고되고 있는 바, Sato 등
30)은 36명의 MMD에서 뇌혈관 재건수술 후 8명(5명에선 minor stroke, 3명에선 일과성 허혈발작)에서 뇌졸중이 나타났으며, 이와같은 합병증을 예 방하기 위하여는 수술중에 정상탄산상태(normocapnia), 정상혈압(normotension), 정상체온(normothermia)을 반 드시 유지하고 충분한 수액공급과 빈혈(anemia) 교정의 필 요성을 강조하였다.
Iwama 등
9)은 124명의 소아 MMD 수술후 21명(16.9%) 에서 뇌허혈성 합병증을 경험하였으며, 이중 11명은 뇌경색, 10명에선 가역성 허혈성 신경학적 결손이었으며, 이들 합 병증이 나타난 환자들은 수술전 일과성 허혈발작(transient ischemic attack, TIA)의 빈도와 수술중이나 후에 45mmHg 이상의 고탄산혈증(hypercapnia)과 관련이 많았다.
그 외에도 수술직후 수술통증으로 심하게 울면서 초래되 는 과호흡상태 때문에도 뇌허혈증상이 나타날 수 있으므로 수술직후 통증관리에 세심한 배려가 필요하다.
최근 Oshima 등
28)은 MMD 환자에서 혈관재건수술시에 사용하는 마취약제에 따라서도 global hyperemia와 연관 된 뇌내절취현상(intracerebral steal phenomenon)으로 인한 허혈성 합병증이 나타날 수 있다고 하였는바, isoflur- ane이 propofol에 비하여 충혈(hyperemia)을 잘 초래한다 고 하였다.
MMD 수술과 관련된 뇌허혈성 합병증을 유발하는데 관 여하는 위험요소들로서는 수술전 TIA의 빈도, 수술중 및 후 저탄산혈증(hypocapnia), 고탄산혈증(hypercapnia), 저혈 압(hypotension)과 MMD의 중증도, 수술방법, 마취제, 탈 수, 혈액량의 부족, 고열(hyperthermia) 등을 들수 있다.
1999년 MMD에 대한 일본연보의 요약
1999년도 일본 보건후생성의 The Research Commi- ttee on Spontaneous Occlusion of the Circle of Willis (Moyamoya Disease)의 초록내용
35)에 의하면;
1) 1999년에 77명의 신환자가 추가되어 이제 등록된 총 환자수는 1,161명이며 그 중에서 1,074명은 definite MMD 이며 이중 남성은 388명, 여성은 686명이다.
2) 출혈성 MMD에서 재출혈이란 MMD에서 가장 나쁜 예후의 원인중 하나이며, Bypass 수술은 허혈성 MMD의 장기간의 예후를 좋게 하는 요소이다.
3) MR 혈관조영상에 나타나는 수술후 뇌혈관 상의 여러 가지 변화들로서는 MMD 혈관의 감소, 경동맥 fork의 폐색, 외경동맥분지의 비후, 중대뇌동맥 원위부에 혈관망의 형성 등이다.
4) MMD의 staging을 하는데 있어서 acetazolamide 반 응을 이용한 SPECT로서 6 stages로 나눌 수 있는 바, 이 는 Suzuki의 혈관조영상에 의한 staging과는 일치하지 않 으나 오히려 임상적 중증도와 관련이 있다.
5) 출혈성 MMD에서도 혈관재건수술이 재출혈의 위험을 의미있게 감소시키는데 기여한다고 보지만, 앞으로 더 많 은 환자에서 장기적인 전향적 조사가 필요하다.
6) 가족적인 MMD에서 염색체 3,6,17번에 어떤 alleles 와의 통계적인 연관성이 있다.
요 약
MMD의 외과적 치료에서 소아에서는 직접 및 간접적인 혈관문합술을 병합하거나 아니면 간접적인 혈관문합술들을 병합하여 시행하는 경향이 있고, 성인에서는 혈관의 직경 이나 MMD의 진행속도로 보아 STA-MCA 문합술과 같 은 직접적인 수술이 많이 활용되고 있으나, 소아나 성인에 서나 모두 공통적으로 보다 넓은 뇌표면에 여러 가지 수술 방법을 병합하는 것이 좀더 광범위한 범위에 뇌혈류 공급 을 증가 시키는데 도움이 된다고 보는 경향이다.
수술의 적응증은 혈관조영상에서의 나타난 질병의 정도 보다는 acetazolamide를 이용한 PET 나 SPECT 검사로 서 뇌혈류 예비량에 기준을 두고 있는 현상이 혈류 역학적 으로도 합리적인 것으로 보인다. 수술후 합병증을 줄이려 면 수술중에 정상탄산, 정상체온, 정상혈압을 유지 시킴과 동시에, 수술직후 통증관리도 철저히 하는 것이 매우 중요 하다.
