메밀에 의한 기관지천식 1예

2배경: 메밀가루(Fagopyrum esculentum)는 우리나라를 비롯한 아시아 일부 국가에서 주로 국수의 재료로서 식용으로 이용되어 이 지역의 중요 한 음식물 알레르겐으로 지목되었던 식품으로 메밀가루에 의한 알레르기는 두드러기/혈관부종부터 아나필낙시스까지 다양한 임상상을 보이 며 그 임상적 중요성이 부각되어 있다. 연자들은 최근 천식, 아토피피부염이 있던 환자에서 메밀가루에 의한 천식발작을 경험하였기에 문헌 고찰과 함께 보고하는 바이다 증례: 15세 남자가 내원 30분전부터 시작된 호흡곤란과 경련을 주소로 응급실을 방문하였다. 과거 천식으로 진단받고 간헐적으로 fluticasone/salmeterol diskhaler를 사용하던 환자는 내원 2시간 전에 메밀이 포함된 학교급식을 먹고, 내원 30분 전부터 호흡곤란 있어서 구급차를 통해서 응급실로 내원하였고 내원 후 2차례의 경련이 있었다. 내원 8년 전에 천식, 아토피 피부염을 진단 받았고, 2년 전에 냉면 먹은 40분 후에 호흡곤란과 두드러기로 본원응급실 내원하여 음식물에 의한 아나필락시스 진단을 받았다. 응급실 내원 당시 혈압 124/96 mmHg, 맥박 142회/분, 호흡수 16/분, 체온 36.6도이었고 의식은 없었다. 청진상 전폐야에 천명음이 들였으며 신경학적 검사상 특이 소견은 없었다. 동맥혈가스 검사에서 pH:7.09, PaO2 66 mmHg, PaCO2 91mHg, 산소포화도 83%이었고, 말초혈액 백혈구수는 16,280/ul (호산구 10.6%), 혈색소 16.6g/dl, 혈소판 232,000/ul이었고, 다른 생화학적 검사와 Brain CT, 흉부단순촬영, 부비동촬영은 정상범위였다. 즉시 기관삽관 후 기계환기를 시작하였고 전신적인 스테로이드와 기관지확장제를 투여 후 중환자실에 입원하였다. 입원 후 시행한 40종의 음식물 항원에 대한 피부단자시험에서 시금치이외에는 밀가루, 호밀가루, 보리, 귀리에 대해서는 음성반응을 보였고, allergen specific immunoglobulin (UniCAP)검사에서 메밀항원에 대한 양성반응(class 3: 13.2 RU/ml)을 보였다. 흔한 흡입성 항원에 대해서는 집먼지진드기 (Dp, Df)에는 강양성, 두드러기 쑥, 쑥 꽃가루, 곰팡이(Aspergillus fumigatus), 오리나무에 대해서 양성을 보였다.(내원 6년 전과 3년 전에 본원 소아과에서 시행한 allergen specific immunoglobulun (UniCAP)검사에서 집먼지 진드기(Dp:Class 5, Df;Class 6), Aspergillus (Class 3), 두드러 기 쑥과 개비듬(Class 2)에 양성반응을 보였다, total IgE는 1,682 KU/L이었다). 중환자실에서 2일간의 기계환기 후 호전되어 병실에서 3일간 입원 후 메밀에 의한 식품알레르기에 대한 교육 후 퇴원(당시 폐기능은 정상이었다)하여 현재는 fluticasone/salmeterol diskhaler로 조절하며 외래 추적 중이다.

A case of propofol-induced oropharyngeal angioedema and bronchospasm

Introduction: Propofol is a substituted isopropylphenol compound that causes a depression in consciousness. Propofol is thought to be a relatively safe intravenous anesthetic and rare cases of anaphylaxis following propofol administration have been reported. Here we report a patient who experienced an anaphylactic reaction with severe oropharyngeal edema and bronchospasm during few minutes after receiving propofol during endoscopic examination. Case report: A 74-year-old woman visited our hospital to undergo esophagogastroduodenoscopy (EGD) for a check-up.
She had an allergy to soybean, but she had no other remarkable medical history. Sedation was induced with 15 mg intravenous propofol. 1 min following administration of propofol, stridor was heard and oxygen saturation fell to 56% on pulse oximetry. We tried to remove the endoscope.
But, the endoscope has stuck in her throat and it was not pulled out. We removed the endoscope by compulsion. After 10 seconds endoscope removal, severe wheezing was heard and her oxygen saturation fell to 56%. Marked swelling of the epiglottis was detected. Immediately 1 mg epinephrine was administered subcutaneously, together with 125 mg methylprednisolone was infused intravenously. Because the patient was not improved in symptom after 1 minute, 1 mg epinephrine was administered intravenously. After 1minute, her oxygen saturation recovered to 98%
and the wheezing was subsided. Skin-prick tests showed immediate reaction for propofol and 20% Intralipid. So we suspected soybean in intralipid, a component of propofol as the cause of this anaphylaxis. She was informed of the results and of the risk for anaphylaxis if re-exposed to propofol or nutritional supplements containing soybean.
