The effects of the allergen immunotherapy with Hop J pollen

Background: Our previous study suggested that the major allergen of Hop J pollen, 10 kDa was intensified with generation of new IgE binding components in recent years. The favorable effects of the 3 years` allergen immunotherapy with Hop J pollen extracts were reported.Purpose:

We observed the effect of allergen immunotherapy on the IgE binding components which were found to increase with environmental changes for 10 years. Methods: Hop J pollens collected in Suwon in 2009 and prepared to allergen extracts for ELISA and immunoblot. The Hop J immunotherapy extracts were provided by Allergopharma( Germany). Conventional immunotherapy was performed for 3 years with serial serum collection in nine patients with allergic rhinitis and asthma. Skin prick tests were performed three times (before, one year and three year) and the skin reactivity was presented as A/H ratio. The changes of serum specific IgE and IgG4 antibodies to Hop J pollen extracts were measured by ELISA. The changes of IgE and IgG4 binding components were compared by immunoblot analysis. Results: The skin reactivity significantly decreased after 3 years. The serum specific IgE levels were not significantly different during 3 year (p=0.953), while specific IgG4 levels increased from 1 year, which increased further at 3 year (p=0.008, p=0.008, respectively). The Immunoblot analysis showed that IgE binding to the10 kDa decreased after 3 years, while IgG4 binding increased to both 10 kDa and additional components including 95,74,18,13 kDa from 1 year. Conclusion: Allergen immunotherapy with Hop J pollen extracts could be effective on the allergenic components which are augmented by environmental changes. 아스피린 불내성 천식과 호산구성 폐렴 양상의 상관성

연구배경: 천식과 동반하여 호산구성 폐렴 양상을 나타내는 질환인 Churg-Strauss 증후군(CSS) 환자 중 적지 않은 사람에서 아스피린 불내성 천식(AIA)이 있다고 보고되었고, 알레르기성 기관지폐 아스페르질루스병(ABPA) 역시 CSS와 관련 있다는 보고도 있어서 AIA 환자들에서 CSS와 ABPA 관련 양상들의 연관성에 대해 조사하였다. 방법: 전남대학교병원 알레르기내과에서 아스피린 등 NSAIDs와 명백히 관련된 천 식증상 병력이 있거나 아스피린 유발시험 양성반응을 보인 AIA환자들과 병력 없이 아스피린 유발시험 음성반응을 보인 천식(ATA)환자들 사이에서 CSS와 ABPA 관련 양상을 비교하였다. 결과: AIA환자 92명은 ATA환자 153명에 비해 여자가 많고 연령이 유의하게 높았다(각 p<0.001). FEV1은 AIA환자에서 유의하게 낮았으나(80.3±1.9% 대 86.4±1.0%, p<0.01) 심한 기도과민성 동반은 유의한 차이가 없었다. 말초 혈액 호산구 수가 >1,000/mm3인 경우는 AIA환자에서 유의하게 많았으나(16.3% 대 7.2%, p<0.05) 혈청 총 IgE치 >417 IU/mL인 경우는 유 의한 차이가 없었다. 집먼지진드기에 대한 혈청 특이 항체 ≥3+는 AIA환자에서 유의하게 적었으나(46.9% 대 64.9%, p<0.05) Aspergillus fumigatus 특이 IgE항체 양성반응과 IgG 침강항체 양성반응에는 유의한 차이가 없었다. CSS환자 분포에도 유의한 차이는 없었다. A.

fumigatus 특이 IgE항체 또는 IgG 침강항체 양성반응을 보인 46예에서 다른 군에 비해 호산구 수 >450/mm3인 경우(58.7% 대 40.8%, p<0.05), 혈청 총 IgE치 >417 IU/mL인 경우(60.9% 대 41.2%, p<0.05), 혈청 총 IgE치 >1,000 IU/mL인 경우(32.6% 대 13.4%, p<0.01)가 각각 유의하게 많았다. 결론: AIA 환자에서 호산구성 폐렴의 특징적인 호산구증다증 양상을 많이 보였으나 A. fumigatus 특이 항체나 CSS와 의 상관성을 발견하기는 어려웠다.
