만성콩팥병 환자에서 오메가-3 지방산 복용유무에 따른 비타민 D 활성의 차이

비타민 D는 간에서 25-hydroxyvitamin D가 되며 다시 콩팥에서 1, 25-dihydroxyvitamin D가 되면서 활성 비타민 D가 된다. 만성콩팥병이 진행 하면 비타민 D를 활성화 시키는 1α-hydroxylase 효소의 활성이 감소되어 1, 25-dihydroxyvitamin D의 수치가 감소한다. 최근 투석환자를 대상 으로 오메가-3 지방산 투여 후 1, 25-dihydroxyvitamin D수치가 증가되었다고 한다. 본 연구의 목적은 투석전 만성콩팥병 환자에서 1, 25-dihydroxyvitamin D 수치를 확인하고 오메가-3 지방산 복용 유무에 따른 1, 25-dihydroxyvitamin D수치의 차이가 있는지 확인 하는데 있다.

본 연구는 후향적조사 연구로 2009년 3월부터 2013년 2월까지 25-hydroxyvitamin D 및 1, 25-dihydroxyvitamin D를 동시에 측정한 20세에서 80세 사이의 환자 중 만성콩팥병 3, 4단계 환자를 대상으로 조사하였다. 오메가-3지방산 복용군은 31예(만성콩팥병 3단계 80.6%) 대조군으로 오메가-3 지방산을 복용하지 않는 군은 32예(만성콩팥병 3단계 81.3%)였다. 25-hydroxyvitamin D 수치가 20 ng/mL 이하인 환자는 73%였고 1, 25-dihydroxyvitamin D의 수치가 25 pg/mL 이하인 환자는 15.9%였다. 오메가-3 지방산 복용군과 대조군의 평균연령(59.8±12.7 vs. 64.3±10.1세), 성별(남자 48.4% vs. 62.5%), 당뇨병의 유병률(25.8% vs. 45.6%), 25-hydroxyvitamin D (16.4±9.0 vs. 21.7±23.1 ng/mL), 인수치, 부갑상선 호르 몬 수치, creatinine (1.63±0.38 vs. 1.75±0.45 mg/dL), 사구체 여과율(42.3±10.9 vs. 40.3±11.0 ml/min/1.73 m2) 및 cystatin C (1.80±0.55 vs.

1.89±0.49 mg/dL) 수치의 의미있는 차이는 없었다. 오메가-3지방산 복용군에서 대조군에 비해 칼슘 (9.1±0.5 vs. 8.8±0.5 mg/dL, p=0.022), 혈색 소(13.4±1.9 vs. 12.2±1.8 g/dL, p=0.015) 및 1, 25-dihydroxyvitamin D (41.3±16.2 vs. 33.7±12.8 pg/mL, p=0.043) 수치가 의미있게 높았다. 대상 환자에서 1, 25-dihydroxyvitamin D의 수치는 사구체여과율(r=0.380, p=0.002) 및 혈색소 수치(r=0.376, p=0.003)와 양의 상관관계를 보였다.

결론적으로 만성콩팥병 3 및 4단계에서 비타민 D가 부족한 환자는 많았지만 활성 비타민 D가 낮은 환자의 빈도는 높지 않았다. 오메가-3 지방

산은 비타민 D의 활성 및 빈혈개선에 도움이 될 것으로 추정되며 향후 전향적인 연구가 필요할 것으로 생각된다.

Background/Aims: Inflammation is an important factor in renal injury. Ferritin as an inflammatory marker has been considered an independent predictor of rapid renal progression in chronic kidney disease (CKD) patients. However, little is known about the relationship between ferritin and residual renal function (RRF) in peritoneal dialysis (PD) patients. Methods: We reviewed the medical records at the Soonchunhyang University Bucheon Hospital in Korea and identified patients who started PD between June 2001 and March 2012. A total of 123 patients were enrolled in the study. Within 1 month after the initiation of PD, RRF was determined by 24-hour urine collection and measured every 6 months.

Clinical and biochemical data at the time of initial 24-hour urine collection were considered as baseline. Results: The reduction rate of RRF were significantly greater in patients with a high ferritin group (Ferritin ≥ 250 ng/mL) compared with those with a low ferritin group (Ferritin

<250 ng/mL) (-1.71±1.36 mL/min/year/1.73m2 vs -0.84±1.63 mL/min/year/1.73m2, p=0.007). Pearson’s correlation analysis revealed that there were significant negative correlations between the baseline serum ferritin level and the reduction rate of RRF (r=-0.219, p=0.015). In multiple linear regression analysis adjusted for other risk factors, the baseline serum ferritin was found to be an independent factor of the reduction rate of RRF (ß=-0.002, p=0.002). Conclusions: This study shows that a higher ferritin level is significantly associated with a more rapid decline in RRF in PD patients.
