Ⅴ. CONCLUSION

The gynecological cancer survivors had pelvic floor dysfunction that affected their quality of life.In this small, short-term pilot study, PFMT integrated with core exercise may improve quality of life of gynecological cancer survivors with pelvic floor dysfunction through increased pelvic muscle strength and enhancing excitatory motor pathway. Larger randomized, long-term trials are needed to determine the effectiveness of the proposed therapeutic approach.

부인과 암 생존자들의 골반 기저 기능장애가 삶의 질에

미치는 영향 조사 및 삶의 질 향상을 위한 신경조정술을

통합한 골반 기저근 재활 프로그램 개발

목적: 부인과 암과 치료에 따른 골반 기저 기능 장애 발생 양상과 경과, 포괄적인 삶의 질 수준을 조사하여 골반기능 장애가 이에 미치는 영향을 분석하고 부인과 암 생존자들의 삶의 질 향상을 위한 골반 기저근 재활 프로그램을 개발하고 이의 효과를 평가하여 지속적인 삶의 질 향상 모델을 제시하고자 하였다.

방법: 34명의 부인과 암 생존자와 16명의 정상 서인 여성을 대상으로 골반 기능 장애 설문지와 삶의 질 평가도구 (EORTC QLQ-C30와 QLQ-CX24)를 이용하여 골반 기능 장애가 삶의 질에 미치는 영향을 분석하고, 전향적 무작위 대조 임상시험을 통하여 부인과 암 수술 이후 골반 기저 운동 프로그램 실시 후 골반기저 기능 설문, 골반 기저 근육 수축력, 운동유발

전위를 비교하였다.

결과: 부인과 암 생존자는 정상 성인 여성에 비하여 삶의 질 특히 신체기능 을 저하시키는 골반 기저 기능 장애가 유의하게 더 많았다. 운동 군에서 대조 군에 비하여 골반기저장애 프로그램을 시행 후 유의하게 골반기저 근육 수축 력이 증가하였으며, 운동 유발 전위의 amplitude 가 증가하였다.

결론: 부인과 암과 치료는 골반 기저 장애를 유발하여 삶의 질을 저하시키 며 골반 기저 재활 프로그램을 통하여 골반 기저 장애가 호전되었다. 이 예비 결과를 통하여 부인과 암 생존자에게 적용 가능한 재활 프로그램을 제안한다.
