I hereby agree and confirm with the below statements:
1. I agree that the Korean Embassy checks for forged financial, official, personal, sponsorship documentation and all related documents on visa applications.
2. I agree that the officials of the Korean Embassy may make inquiries with the relevant institutions regarding this application. The Korean Embassy and the relevant institutions will release personal information in accordance with the relevant laws and regulations in Ghana to the Ghana Police CID. I further agree that passport and forged or stolen documents will not be returned and will be handed over to the Ghana Police CID.
3. I did not pay/offer/supply any extra money to any Ghanaian nor Korean nor whoever for the purpose of facilitating the Korean visa, apart from the stipulated visa fee to the Embassy.
4. I am aware that the submission of forged documents is a criminal offence in Ghana and that should forged documents be identified in this application, I am personally liable to prosecution by Ghana Police. I am also aware that penalties may be imposed, if a conviction is found and may involve imprisonment.
